projects will be submitted to a committee for final approval. This form must be completed and
returned to your senior English teacher by Friday, August 17, 2012. You will not be allowed to
change your topic after October 12, 2012.

The Senior Project is subject to the plagiarism policies set forth by the district and Creekview High
School.

1) Describe the overall topic of your Senior Project.
The overall topic of my Senior Project is to plan out, market for, and participate in a health fair that
will take place at my church – Hickory Flat UMC.


2) What is your previous experience in this area of study?
Although I do not have much experience in planning events, I am around medical procedures often. I
am a football student trainer for Creekview. I have also attended the UGA Athletic Training Camp
where I was certified in CPR and taping. I am also well organized and hope that will help me in
planning the fair.


3) To what career will you connect this project (required)?
This project can be connected to any job in the medical field. While the planning for the project would
link closely to an administrative role in a hospital (such as the dean of medicine), the participation
could be associated with nursing or health education.

4) Describe what you are proposing to do for your Product step-by-step and in detail. Write on the
back of this sheet if necessary.

1.) Meet with my facilitator to decide which drug companies, physicians, specialists, workout centers,
etc. would be best suited to the target audience. The target age group will most likely be middle-aged
to elderly adults.
2.) Contact or assign people to help contact these companies and offices to confirm that they will be
able to attend the health fair. These companies would either set up a booth or give a small
presentation in their area of expertise.
3.) Visit Dr. Donna Kremer who has agreed to help with the production of the health fair. My
facilitator and I will need to ask her for efficient supplies and possibly a small grant to help with the
cost of the fair.
4.) Work out the logistics of the facility with my facilitator. This would include making sure every
booth has enough space and every presentation has suitable technology and a suitable space.
5.)Advertise the health fair by making a website, creating flyers, and publishing an ad in the
newspaper so people in the community will be informed.
6.) Create a small presentation of type 1 diabetes that will be shown at the health fair with the
assistance of medical staff from KSU.
7.) Attend the health fair and give my presentation.
5) How is this project a “learning stretch” and a challenge for you?
This project is a stretch for me because I will need to work efficiently behind the scenes to complete
this health fair. I know a lot of people are counting on me and I will need to work extremely hard to
not disappoint them. I will also meet many new people in the medical field which puts me out of my
comfort zone. But I know that this will help me as I move into a possible career as a nurse.


6) What are your estimated costs for completing the Product, and how will you fund these costs?
The fair will cost a few hundred dollars. While most of the companies and physicians are coming for
free, it will take some money to make the facility useable. The Health Ministry at my church will fund
the fair but we hope to get a small grant from Wellstar to supplement the costs.


7) Who have you chosen (or are considering choosing) as your Project Facilitator?
Diane Keen


8) What is this person’s expertise in this field?
Diane Keen is a nurse who is currently a full time nursing teacher at KSU. She will be attending KSU
once again to get her doctorate over the next few years. She has worked in a hospital and in inpatient
care. She also serves as the nurse for all of our youth events that take place at the church. Diane
started the Health Ministry at my church and hopes I can continue to help it grow.
