The document describes the development and validation of the VI-Med® tool for medication review in Vietnamese hospitals. The tool consists of 3 forms to collect patient information, conduct medication review, and document pharmacist interventions. Six clinical pharmacists used the tool to classify sample drug-related problems and pharmacist interventions. Their classifications showed good to excellent agreement according to Cohen's Kappa scores. A survey found the pharmacists were satisfied with the tool's usefulness, ease of use, and that it can be used to support medication review in clinical practice. The VI-Med® tool is validated and ready to use to standardize the medication review process in Vietnam.
