The document provides guidelines for classroom assessment and grading in the Philippine K-12 education system. It outlines a standards-based grading system where learners are assessed based on written work, performance tasks, and quarterly assessments. Scores from these components are converted to percentages and weighted according to their importance. The weighted scores are then transmuted to determine quarterly and final grades. Progress is reported to parents through quarterly report cards and parent-teacher conferences.

13. DepEd Order No. 8, s. 2015
POLICY GUIDELINES CLASSROOM
ASSESSMENT for K-12
14. What is a grading system?
K to 12 uses a standards and
competency-based grading system
All grades are based on the raw scores.
Learners are graded on (1) WRITTEN
WORK, (2) PERFORMANCE TASK and
(3) QUARTERLY ASSESSMENT
15. How is learner progress recorded and
computed?
For kindergarten
Checklist – the teacher will indicate if the
child demonstrate knowledge or perform
the task expected
Anecdotal records – teachers will be able to
describe learners behavior, attitude and
effort in school work.
16. How is learner progress recorded and
computed?
For grades 1-12
Learners are graded on these components
(1) WRITTEN WORK
(2) PERFORMANCE TASK
(3) QUARTERLY ASSESSMENT
17. Steps in computing Grades
Step 1
Grades from all student work are added up
from each components.
These raw scores have to be converted to a
Percentage score.
18. Steps in computing Grades
Step 2
The sum of each component is converted to the
Percentage score.
𝑃𝑒𝑟𝑐𝑒𝑛𝑡𝑎𝑔𝑒 𝑆𝑐𝑜𝑟𝑒 𝑃𝑆 =
𝑡𝑜𝑡𝑎𝑙 𝑟𝑎𝑤 𝑠𝑐𝑜𝑟𝑒
ℎ𝑖𝑔ℎ𝑒𝑠𝑡 𝑝𝑜𝑠𝑠𝑖𝑏𝑙𝑒 𝑠𝑐𝑜𝑟𝑒
x 100
19. Steps in computing Grades
Step 3
Percentage scores are then converted to
Weighted scores to show the importance of
each components in promoting learning.
𝑊𝑒𝑖𝑔ℎ𝑡𝑒𝑑 𝑠𝑐𝑜𝑟𝑒 𝑊𝑆 =
𝑃𝑒𝑟𝑐𝑒𝑛𝑡𝑎𝑔𝑒 𝑆𝑐𝑜𝑟𝑒 𝑥 𝑊𝑒𝑖𝑔ℎ𝑡 𝑜𝑓 𝑡ℎ𝑒 𝐶𝑜𝑚𝑝𝑜𝑛𝑒𝑛𝑡

22. Steps in computing Grades
Step 4
The sum of the weighted score in each
component is the INITIAL GRADE.
This will be transmuted using the
transmutation table (see Appendix B)
to get the Quarterly Grade.
23. Steps in computing Grades
Step 5
The QUARTERLY GRADE for each
learning area/subject is written in
the report card of the student.
24. How are grade computed at the end of
the school year?
For kindergarten
There are no numerical grades
The description of the learners
progress are represented using
checklist and portfolios.
25. How are grade computed at the end of
the school year?
For grade 1-10
Average of the Quarterly Grades (QG)
produces the Final Grade
𝐹𝑖𝑛𝑎𝑙 𝑔𝑟𝑎𝑑𝑒 =
1𝑠𝑡 𝑄𝐺+2𝑛𝑑 𝑄𝐺+3𝑟𝑑 𝑄𝐺+4𝑡ℎ 𝑄𝐺
4
26. How are grade computed at the end of
the school year?
Example
1QG = 88
2QG = 86
3QG = 83
4QG = 88
FINAL GRADE = ?
27. How are grade computed at the end of
the school year?
For grade 1-10
The General Average is computed by
dividing the sum of all final grades by
the total number of learning area.
𝐺𝑒𝑛𝑎𝑟𝑎𝑙 𝐴𝑣𝑒𝑟𝑎𝑔𝑒 =
𝑆𝑢𝑚 𝑜𝑓 𝑓𝑖𝑛𝑎𝑙 𝑔𝑟𝑎𝑑𝑒𝑠 𝑜𝑓 𝑎𝑙𝑙 𝑙𝑒𝑎𝑟𝑛𝑖𝑛𝑔 𝑎𝑟𝑒𝑎𝑠

29. How are grade computed at the end of
the school year?
For grade 11-12
The two quarters determine the Final
Grade in a semester.
30. How is the learners progress reported?
The summary of the learner progress is
shown quarterly to parents or
guardians through a parent-teacher
conference, in which the report card is
discussed.
31. How is the learners progress reported?
