This study compared the effects of task specific strength training (TSST) and resistance training (RT) on lower limb strength and function in 30 hemiparetic patients. Patients were randomly assigned to receive either TSST or RT for 45 minutes, 3 times per week for 4 weeks. Both groups showed improvements in timed up and go test (TUG) scores and hand held dynamometer (HHD) measurements of muscle strength after the intervention. However, the group that received TSST showed greater improvements in lower limb strength and function compared to the group that received RT.

The study aims to 1) use transcranial direct current stimulation (tDCS) to identify the influence of each motor cortex hemisphere on the affected limb for each subgroup and 2) determine how targeted therapies differentially enhance motor control for each subgroup. The significance is that current treatments do not consider injury timing, but recent evidence suggests differences in motor pathways between subgroups that could inform more tailored therapies. The innovation is applying tDCS and targeted therapies known to benefit stroke patients to CP subgroups to exploit pathway differences identified through tDCS.

This pilot study evaluated the effects of 8 weeks of teriparatide (TPTD) versus placebo treatment on bone biomarkers, structure, and stress fracture healing in premenopausal women with lower-extremity stress fractures. The study found that TPTD treatment led to greater increases in bone formation markers and a larger "anabolic window" compared to placebo. TPTD treatment also showed improvements in bone structure at weight-bearing sites on imaging. A higher percentage of stress fractures showed improvement or healing with TPTD versus placebo on MRI, though the results were not statistically significant. The study provides preliminary evidence that TPTD may help hasten stress fracture healing in premenopausal women.

The trials involved the use of manual therapy techniques like mobilization combined with therapeutic exercises. The results showed that manual therapy along with exercises was effective in improving function, range of motion, and decreasing pain and disability levels for patients with cervical radiculopathy. High quality randomized controlled trials are still needed to establish clear treatment protocols.

A minimum of 30 subjects will be divided into two groups - group A will receive cervical rotatory manipulation along with other treatments, while group B will only receive other treatments. Background and Objective: According to world Health Organization (WHO) stroke is defined as œrapidly developing clinical sign of focal (or global) disturbance of cerebral function lasting more than 24 hours or leading to death, with no apparent cause other than vascular origin.1 Focal neurological deficits must persists for at least 24 hours, motor deficits are characterized by paralysis (hemiplegia) or weakness (hemiparesis), typically on the side of the body opposite site of lesion.Materials and Methods: The study was performed among 30 patients of both genders, aged 45-60 years. Subjects were selected on the basis of inclusion criteria and randomly divided into two groups by convenience sampling and allocating alternate patient group A and group B, 15 in each group. Group A was treated with Repetitive Facilitation Exercises (RFE), Group B was treated with Hand Arm Bimanual Intensive Training (HABIT). Baseline assessments were taken using WMFT and FMA and data was analyzed.Results: The groups showed significant differences in WMFT and FMA variables. But on comparing the mean of both the groups: there was no significant difference between both the groups. Conclusion: This study concluded that RFE and HABIT both are effective in treatment of patients with hemiparesis. Abstract: Low back pain is considered one of the commonest condition in the western and industrialized
countries. It is estimated that up to 50% of adults experience low back pain during their life span. People of all
age group can be effected by this menace irrespective to their gender and quality of life. It has become one of
the leading causes for the visit to physician thus also puts a heavy burden on the currency of the country.
Physiotherapy is the most widely used form of treatment adopted for gaining relief from low back pain. The
exercises include stretching, strengthening, range of motion exercises, McKenzie therapy and core stability
exercises other techniques like muscle energy technique etc. It has been concluded in various studies core
stability exercises and muscle energy technique are beneficial in low back pain patients but comparison of their
effect needs to be established to provide early and better relief from the disability. Therefore objective of the
study was to compare the effect of core stability exercises and muscle energy techniques on low back pain
patients. 60 subjects aged 18 – 45 years with low back pain were made part of the study based on inclusion and
exclusion criteria and were then divided into three groups named A, B and C. Group A received core stability
exercise and conventional physiotheraphy and group B received muscle energy techniques and conventional
physiotherapy. The exercise program was given on alternate days with a total of 24 sessions and progression of
the activity was made within the tolerance of the patient. Pre and post treatment readings were taken of pain,
ROM and quality of life scale. Results were analyzed using paired, unpaired t- test and ANOVA. Results showed
that there is significant effect on pain, ROM and quality of life scale in the three groups but group A was
clinically more significant than the other groups. The study concluded that patients with low back pain are
benefitted more by core stability exercises. So, core stability exercises should be practiced more.

Abstract
Grip strength has a specific role in ergonomics especially for hand tool using. In ergonomics a hand tool must be designed such a way that provide a better fit for the user, hence it reduces the risk of potential injuries associated with the use of hand tool. Grip strength decreases in repetitive hand activities, and that it will recover after an adequate rest period. This paper presents the relationship between isometric grip strength and recovery in addition to the influence of potential anthropometric factors on grip strength. 20 male and 20 female subject’s age range from 20 to 51 years were selected in order to evaluate dominant hand grip strengths. Factors assessed for potential association with grip strength were; demographics such as age and gender; body constructs such as height and weight; upper extremity such as the linear dimensions of elbow to wrist length, hand length and hand breadth. Linear regression analysis was performed in order to identify influencing factors independently associated with grip strength. Pearson product correlation was performed to identify the nature of relationship between isometric grip strength and anthropometric parameters. Through descriptive statistics based on greatest mean grip strength, optimal handle position has been found in position-3 (3.5 cm) for male and position-2 (3.0 cm) for female. Mean grip strength of male has been found greater than female. Grip strengths of male and female in dominant hands are found to decrease with aging, and are significantly different between male and female. Z-test indicates that there are no significant differences on the rate of recovery at each point in time between groups. Linear regression analysis indicates that weight and hand breadth in male; age and weight in female have a significant effect on grip strength.

This literature review examines the effect of aerobic exercise on walking capacity in subjects with parkinsonism. Seven studies that utilized aerobic exercises like treadmill training and body weight supported treadmill training with subjects with Parkinson's disease were reviewed. The results across the studies found that aerobic exercise improved walking parameters like speed, stride length, home ambulation, and reduced movement initiation time. The literature review concluded that aerobic exercises can improve walking capacity in patients with parkinsonism.
This case study examined the effects of a 16-week home exercise program combining mental imagery, constraint-induced movement therapy, mirror therapy, and proximal scapular strengthening on a patient 26 years post-stroke. Assessments showed improvements in upper extremity strength, scapular positioning, and motor function scores that exceeded minimal detectable change thresholds. The patient also reported greater satisfaction with upper extremity function. This provides preliminary evidence that neuroplastic changes and functional gains may be possible even in chronic stroke cases through continued skilled therapy. Further research is still needed but this study contributes to understanding neuroplasticity in chronic stroke patients.

This study compared the effectiveness of three treatment modalities for idiopathic adhesive capsulitis of the shoulder: 1) conservative treatment including physical therapy, 2) intra-articular steroid injections plus physical therapy, and 3) arthroscopic capsular release plus physical therapy. The study found that arthroscopic capsular release resulted in significantly better improvement in external shoulder rotation compared to conservative treatment alone. However, overall functional outcomes as measured by a shoulder rating questionnaire were similar across groups. The study concluded that conservative treatment remains an effective first-line option for adhesive capsulitis, though arthroscopic release may provide faster recovery of external rotation.

1) Posterior glenohumeral joint capsule contracture alters normal shoulder joint mechanics and is commonly seen in throwing athletes due to repetitive tensile forces on the tissue.
2) The posterior capsule limits posterior translation of the humeral head and tightens with internal rotation and horizontal adduction of the shoulder. Increased stiffness of the posterior capsule is thought to alter humeral head movement and lead to shoulder pathology.
3) At the cellular level, fibroblasts in the capsule sense mechanical loading and respond by altering gene expression, leading to increased collagen synthesis and proliferation over time consistent with Wolff's law, eventually resulting in a stiffened posterior capsule.

Abstract— Respiratory complications are major sources of morbidity and mortality in spinal cord injury (SCI). Improvement in expiratory muscle strength may be associated with improved cough and clearance of secretions. So that expiratory muscle training are advised in SCI cases to improve expiratory muscle strength. This study was conducted to compare the effect of Resistance and Sham training on pulmonary function test in spinal cord injury cases. It was found the mean value of all the parameters of PFT of Sham training was found to increase from baseline to 1st and 2nd follow up. This difference was found significant in all the parameters of PFT at 2nd follow up, but FEV1 and PIFR of 1st follow up (p value < 0.05) only. Likewise, it is also observed that the mean value of all the parameters of PFT of test group was found to increase from baseline to 1st and 2nd follow up. This difference was found highly significant (p value <0.001) in all the parameters of PFT at 1st and 2nd follow up. So it can be concluded that however both the type of training improve PFTs but the improvement of high resistance training was significantly more than they were for sham training.
This study evaluated the effectiveness of Osteobios and Guna-Fem supplements for treating early postmenopausal osteoporosis compared to calcium and vitamin D supplements. 70 postmenopausal women with osteoporosis were divided into two groups. The group treated with Osteobios and Guna-Fem showed greater reductions in pain levels, improvements in quality of life, and decreases in bone resorption markers compared to the calcium/vitamin D group. The results suggest Osteobios and Guna-Fem may be effective alternative or additional therapies for treating osteoporosis and improving patients' quality of life.
This study compared the effectiveness of low-dose aspirin (81 mg twice daily) to high-dose aspirin (325 mg twice daily) for preventing venous thromboembolism (VTE) following total joint arthroplasty. The study included over 4,600 patients undergoing primary total joint replacement who received either low-dose or high-dose aspirin for 4 weeks post-operatively. The results found no significant difference in VTE rates between the low-dose and high-dose aspirin groups, indicating low-dose aspirin is not inferior to high-dose aspirin for VTE prophylaxis after total joint arthroplasty.

Work-related Musculoskeletal Disorders (MSD) are one of the most common occupational diseases of offi ce workers. The aim of this study is investigating the prevalence of these disorders, determining the risk factors of occupational diseases and providing control strategies. The aim of this study is evaluating the prevalence of these symptoms among oil refinery staff. The studies conducted 200 administrative staff, 150 were male and 50 were female. It was observed that most musculoskeletal symptoms were in the neck (50%),knee (42.5%), shoulders (38.5%), the hip (10%) and lower arm (8.5%). To investigate the risk factors of musculoskeletal symptoms, we use the new methods of RULA and REBA so that we can take the necessary action to reduce the musculoskeletal symptoms and prevent potential dangers.
Practitioner Summary: This study was conducted to evaluate the prevalence of musculoskeletal disorders among one company of Iran’s oil refineries staff. Basically, because refinery workers need to do a lot of technical and physical work, therefore they have many musculoskeletal disorders. This study suggests that, with these disorders, there should be some necessary measures.
Effectiveness of physiotherapy exercise after knee arthroplasty for oa.FUAD HAZIME
This systematic review evaluated the effectiveness of physiotherapy exercise after knee arthroplasty for osteoarthritis. The review included 6 randomized controlled trials that compared physiotherapy exercise interventions to usual care or different exercise interventions after discharge. The review found small to moderate benefits of functional exercise for function, range of motion, and quality of life 3-4 months after surgery. However, benefits were no longer evident at 1 year, suggesting no long term benefit of physiotherapy exercise after knee arthroplasty.

exercise and conventional physiotheraphy and group B received muscle energy techniques and conventional
physiotherapy. The exercise program was given on alternate days with a total of 24 sessions and progression of
the activity was made within the tolerance of the patient. Pre and post treatment readings were taken of pain,
ROM and quality of life scale. Results were analyzed using paired, unpaired t- test and ANOVA. Results showed
that there is significant effect on pain, ROM and quality of life scale in the three groups but group A was
clinically more significant than the other groups. The study concluded that patients with low back pain are
benefitted more by core stability exercises. So, core stability exercises should be practiced more.

Abstract
Grip strength has a specific role in ergonomics especially for hand tool using. In ergonomics a hand tool must be designed such a way that provide a better fit for the user, hence it reduces the risk of potential injuries associated with the use of hand tool. Grip strength decreases in repetitive hand activities, and that it will recover after an adequate rest period. This paper presents the relationship between isometric grip strength and recovery in addition to the influence of potential anthropometric factors on grip strength. 20 male and 20 female subject’s age range from 20 to 51 years were selected in order to evaluate dominant hand grip strengths. Factors assessed for potential association with grip strength were; demographics such as age and gender; body constructs such as height and weight; upper extremity such as the linear dimensions of elbow to wrist length, hand length and hand breadth. Linear regression analysis was performed in order to identify influencing factors independently associated with grip strength. Pearson product correlation was performed to identify the nature of relationship between isometric grip strength and anthropometric parameters. Through descriptive statistics based on greatest mean grip strength, optimal handle position has been found in position-3 (3.5 cm) for male and position-2 (3.0 cm) for female. Mean grip strength of male has been found greater than female. Grip strengths of male and female in dominant hands are found to decrease with aging, and are significantly different between male and female. Z-test indicates that there are no significant differences on the rate of recovery at each point in time between groups. Linear regression analysis indicates that weight and hand breadth in male; age and weight in female have a significant effect on grip strength.

This study compared the incidence of neurobehavioral side effects of levetiracetam versus phenytoin in patients with traumatic brain injury (TBI). In a prospective observational study of 100 TBI patients treated with either levetiracetam or phenytoin, researchers found:

This literature review examines the effect of aerobic exercise on walking capacity in subjects with parkinsonism. Seven studies that utilized aerobic exercises like treadmill training and body weight supported treadmill training with subjects with Parkinson's disease were reviewed. The results across the studies found that aerobic exercise improved walking parameters like speed, stride length, home ambulation, and reduced movement initiation time. The literature review concluded that aerobic exercises can improve walking capacity in patients with parkinsonism.
This case study examined the effects of a 16-week home exercise program combining mental imagery, constraint-induced movement therapy, mirror therapy, and proximal scapular strengthening on a patient 26 years post-stroke. Assessments showed improvements in upper extremity strength, scapular positioning, and motor function scores that exceeded minimal detectable change thresholds. The patient also reported greater satisfaction with upper extremity function. This provides preliminary evidence that neuroplastic changes and functional gains may be possible even in chronic stroke cases through continued skilled therapy. This study compared the effectiveness of three treatment modalities for idiopathic adhesive capsulitis of the shoulder: 1) conservative treatment including physical therapy, 2) intra-articular steroid injections plus physical therapy, and 3) arthroscopic capsular release plus physical therapy. The study found that arthroscopic capsular release resulted in significantly better improvement in external shoulder rotation compared to conservative treatment alone. However, overall functional outcomes as measured by a shoulder rating questionnaire were similar across groups. The study concluded that conservative treatment remains an effective first-line option for adhesive capsulitis, though arthroscopic release may provide faster recovery of external rotation.

2) The posterior capsule limits posterior translation of the humeral head and tightens with internal rotation and horizontal adduction of the shoulder. Increased stiffness of the posterior capsule is thought to alter humeral head movement and lead to shoulder pathology.
3) At the cellular level, fibroblasts in the capsule sense mechanical loading and respond by altering gene expression, leading to increased collagen synthesis and proliferation over time consistent with Wolff's law, eventually resulting in a stiffened posterior capsule.

Abstract— Respiratory complications are major sources of morbidity and mortality in spinal cord injury (SCI). Improvement in expiratory muscle strength may be associated with improved cough and clearance of secretions. So that expiratory muscle training are advised in SCI cases to improve expiratory muscle strength. This study was conducted to compare the effect of Resistance and Sham training on pulmonary function test in spinal cord injury cases. It was found the mean value of all the parameters of PFT of Sham training was found to increase from baseline to 1st and 2nd follow up. This difference was found significant in all the parameters of PFT at 2nd follow up, but FEV1 and PIFR of 1st follow up (p value < 0.05) only. Likewise, it is also observed that the mean value of all the parameters of PFT of test group was found to increase from baseline to 1st and 2nd follow up. This difference was found highly significant (p value <0.001) in all the parameters of PFT at 1st and 2nd follow up. So it can be concluded that however both the type of training improve PFTs but the improvement of high resistance training was significantly more than they were for sham training.
This study evaluated the effectiveness of Osteobios and Guna-Fem supplements for treating early postmenopausal osteoporosis compared to calcium and vitamin D supplements. 70 postmenopausal women with osteoporosis were divided into two groups. The group treated with Osteobios and Guna-Fem showed greater reductions in pain levels, improvements in quality of life, and decreases in bone resorption markers compared to the calcium/vitamin D group. The results suggest Osteobios and Guna-Fem may be effective alternative or additional therapies for treating osteoporosis and improving patients' quality of life.
This study compared the effectiveness of low-dose aspirin (81 mg twice daily) to high-dose aspirin (325 mg twice daily) for preventing venous thromboembolism (VTE) following total joint arthroplasty. The study included over 4,600 patients undergoing primary total joint replacement who received either low-dose or high-dose aspirin for 4 weeks post-operatively. The results found no significant difference in VTE rates between the low-dose and high-dose aspirin groups, indicating low-dose aspirin is not inferior to high-dose aspirin for VTE prophylaxis after total joint arthroplasty.

Work-related Musculoskeletal Disorders (MSD) are one of the most common occupational diseases of offi ce workers. The aim of this study is investigating the prevalence of these disorders, determining the risk factors of occupational diseases and providing control strategies. The aim of this study is evaluating the prevalence of these symptoms among oil refinery staff. The studies conducted 200 administrative staff, 150 were male and 50 were female. It was observed that most musculoskeletal symptoms were in the neck (50%),knee (42.5%), shoulders (38.5%), the hip (10%) and lower arm (8.5%). To investigate the risk factors of musculoskeletal symptoms, we use the new methods of RULA and REBA so that we can take the necessary action to reduce the musculoskeletal symptoms and prevent potential dangers.
Practitioner Summary: This study was conducted to evaluate the prevalence of musculoskeletal disorders among one company of Iran’s oil refineries staff. Basically, because refinery workers need to do a lot of technical and physical work, therefore they have many musculoskeletal disorders. This study suggests that, with these disorders, there should be some necessary measures.

This systematic review evaluated the effectiveness of physiotherapy exercise after knee arthroplasty for osteoarthritis. The review included 6 randomized controlled trials that compared physiotherapy exercise interventions to usual care or different exercise interventions after discharge. The review found small to moderate benefits of functional exercise for function, range of motion, and quality of life 3-4 months after surgery. However, benefits were no longer evident at 1 year, suggesting no long term benefit of physiotherapy exercise after knee arthroplasty.

Life sciences regulatory authorities in North America, Europe and Japan have been developing standards for drug and biologic registration submissions in electronic format for over ten years. The current version of the format—electronic Common Technical Document (eCTD)—has been the recommended format since 2005 and is on the way to becoming mandatory. This presentation will provide an overview of best practices to enable a sponsor to prepare a compliant eCTD for regulatory authority review.
A California biotech submitted an eCTD to the US FDA in summer 2005. This company used commercial-off-the-shelf publishing software, experienced consultants, and in-house staff to compile this eCTD. This drug was the biotechs first candidate for commercialization. The biotech was partnered with big Pharma to assist with sales and marketing after the FDA authorized the drug for marketing. The biotech hired a sales force of nearly 200 in anticipation of FDA approval.
The FDA issued a refuse-to-file on this eCTD in fall 2005, due to problems with the PDF files and navigability of the content of the submission. The company’s market capitalization dropped 50% overnight. The company had to recompile the submission, by reworking the source files and rebuilding the XML backbone. The new version of the eCTD was resubmitted in early 2006. The FDA accepted the resubmission for review.
However, in May 2006 the FDA issued an approvable letter, that meant the drug could be approved in the future if certain conditions were met (meaning yet another delay in being able to market the product, plus additional expense to conduct further clinical studies). In June, 2006, the big pharma company terminated the partnership with the biotech. In July 2006, the biotech laid off their sales force. In August, 2006, the biotech laid off 100 more employees. In September 2006, the biotech had a meeting with FDA to discuss what additional studies and analysis was needed. Finally, in January 2007, the biotech announced plans to resubmit its NDA by end of second quarter 2007.

Each group receives occupational therapy intervention for 4 weeks, 5 sessions each session will be 50 minutes in a week. In group A subjects received 20 minutes of mirror therapy and group B subjects received 20 minutes of modified CIMT for 20 minutes respectively in addition to 30 minutes of conventional occupational therapy.OUTCOME MEASURES Action Research Arm TestRESULT The current study has shown that there is no significant difference improvement of hand function in mirror therapy with conventional occupational therapy and mCIMT with conventional occupational therapy. This randomized clinical trial compared the effectiveness of anterior versus posterior glide joint mobilization techniques for improving shoulder external rotation range of motion in patients with adhesive capsulitis. Twenty subjects with primary adhesive capsulitis and external rotation deficits were randomly assigned to receive either anterior or posterior glide mobilization plus therapeutic ultrasound and exercise over six sessions. Shoulder external rotation range of motion was measured initially and after each session. The group receiving posterior glide mobilization showed significantly greater improvement in external rotation range of motion compared to the group receiving anterior glide mobilization. Both groups had significant decreases in pain.

This study compared the effects of range of motion (ROM) exercises and massage on muscle strength in critically ill patients admitted to intensive care units (ICUs). Ninety conscious ICU patients were randomly assigned to receive either ROM exercises, massage, or routine care (control group) once daily for seven days. Muscle strength was measured before, during, and after the intervention period using a handheld dynamometer. The results showed that muscle strength in the ROM exercise and massage groups increased after the intervention, while it decreased in the control group. The study concluded that both ROM exercises and massage were effective at increasing muscle strength in critically ill ICU patients.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
