Parents’ and carers’ views at transition
Main concerns at transition:
• The young person’s ability to manage his / her life
• A lack of progression or meaningful daytime occupation
• Increased vulnerabilities associated with independence
• The threat of harm and police involvement arising from behaviour problems
• Poor or worsening mental health
• Parent’s own ability to continue the caring role

Improving frontline practice with young people through training in developmentally appropriate services and person-centred practice Why • Young people will get support tailored to them which they are more likely to engage positively with • Supports staff in their roles to enable young people to focus on their goals and hopes for the future and to make decisions • Gives a personalised framework focused on outcomes that professionals from all services can work with How • Add to staff development plans and plans for team training • Review assessments across services – are they person centred? • All staff to take an holistic view to plans eg how is good health care supporting educational attainment or independence • Staff with specific expertise run sessions with other teams to share approaches • Use young people’s views on their experience to improve planning of assessment and reviews 14

Young people disengaged or not eligible for services Why • Ensure all young people get support when they need it • Avoid more complex support or needs developing in the future • Inform future strategic planning and service design How • Share data on young people in transition to help identify those who are disengaging • Use transition planning to ensure young people understand the impact of taking risks eg in managing their own condition • Make sure young people and families / carers know about the Local Offer and latest information on available support • Examine use of technology eg consultation via Skype and sharing information using social media 15

This webinar will: outline key recommendations from the guideline as well as providing an overview of good practice in transitions provide a brief overview of the importance of ‘good transitions’ enable participants to have a good understanding of the NICE Guideline recommendations  provide an insight into the development of the NICE guideline “Transition from children’s to adult’s services”. Participants will:   Have an understanding of challenges and good practice around transition to adult services   Understand the potential links between the guideline and their practice, so will understand why the guideline is important, as well as to understand barriers/enablers to improving local practice   Be able to articulate the key recommendations of the guideline and their potential to inform practice.
