This document describes a telehealth program called TeleTeam Care that aims to improve care for uncontrolled diabetic patients in underserved rural communities. The program provides a team-based care approach using telehealth to deliver specialty services like nutrition therapy, diabetes management, pharmacotherapy, and behavioral therapy to patients in rural primary care practices. The goals are to lower HbA1c levels, manage weight and behavioral health issues like depression and diabetes distress, and improve self-care behaviors. Initial results showed reductions in HbA1c and stable weight loss. The program aims to expand its patient base and enhance screening to help more patients receive specialty care.
