This study evaluated the diagnostic utility of modified Faine's criteria for diagnosing leptospirosis. The study analyzed 100 patients presenting with fever in a hospital in Bengaluru, India. Clinical, epidemiological and laboratory findings were collected according to modified Faine's criteria. Laboratory tests included dark field microscopy of blood and urine, culture, and serological tests (IgM Leptocheck, IgM ELISA, MAT). Of the tests, MAT was considered the gold standard. The positive predictive values of the screening tests and modified Faine's criteria were calculated using MAT as reference. The positive predictive values ranged from 6.5% to 21%.
