Urbanization is rapidly increasing neonatal mortality rates in Bangladeshi slums. A study showed neonatal mortality was over twice as high in slums compared to non-slum areas. BRAC's Manoshi program aims to combat this issue by training community health workers to provide essential newborn care and refer patients in slums to health facilities. ClickDiagnostics has partnered with BRAC to develop a mobile health system to help community health workers more efficiently track patients, prioritize at-risk cases, and improve health outcomes in slums. An initial pilot project suggests the mobile system can significantly increase community health workers' work efficiency and quality of care.

For the Sri Lankan health sector, the performance management definition is relatively recent. For almost two decades, Sri Lanka has been introducing health sector reforms. The reforms included implementing public sector results oriented management and the decentralisation of health care workers management from central to local governments. However, to propose strategies for improvement, this examination analysed the application of Public Health Midwives PHM , performance management. The brief was a descriptive survey conducted in Sri Lanka and used quantitative review approach. Moreover, the review indicates that performance management is limited to the state health sector, while there have been deceptions in its application. In setting performance goals, there were inadequacies and performance management preparation were hardly performed. Although many Public Health Midwives PHM had job descriptions, both Public Health Midwives PHM and authorities were not identified and aware of the performance metrics and standards as per the WHO. Besides, the timetables for performance reviews have not always been followed. There were limited opportunities for career advancement, insufficient input on performance and inadequate mechanisms for compensation. Public Health Midwives PHM performance management is inadequately carried out in most of the district. A central component of efforts to enhance the results of the health sector is performance management. However, as Sri Lanka advances, the primary health system can provide vital health expertise in rural areas and maintain essential health workers at the grassroots level in remote parts of the country. The scheme has dramatically reduced the worlds maternal and child mortality and helped fill the rural health vacuum. However, by allocating the appropriate amount of funds through state expansionary fiscal policy, it is possible to enhance the standard of training and number of a Public Health Midwife PHM to the nations wellbeing. The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

Mobile Maternal, Newborn, and Child Health (MNCH) with BRAC in Bangladesh

2. Urbanization is happening rapidly in Bangladesh and by 2015, it is estimated that
out of 50 million, 21 million people will be living in slums1. Although healthcare is
available, neonatal deaths are higher in slums than in non-slum areas.

Maternal, newborn and child health (MNCH) has been a major challenge to
overcome in Bangladesh, and the figure is significantly worse in slum areas. A study
showed that between 2002 and 2006, neonatal mortality was 43.7 per 1000 live births
in slums compared to 20.1 per 1000 live births in non-slum areas2.

BRAC has been combating the issue of maternal, newborn and child health in urban slums through
Manoshi program since 2007, funded by the Bill and Melinda Gates Foundation. CHWs are trained to keep
track of all birth records, provide essential newborn care, and refer mother and child to pre-selected health
facilities in the case of complication.
5. CHWs walk door to door and visit 30-45 households daily. They had been manually
recording all pregnant related information on the register book or health card. Patient
information then waited until the end of each month to be collected and submitted.

ClickDiagnostics has
partnered with BRAC
Manoshi to integrate mobile-
phone based healthcare
system for a more cost-
efficient, resource optimizing
and financially sustainable
healthcare model. We
launched our first pilot
project in July 2009 for the
duration of 7.5 months. We
were able to alleviate
several bottlenecks in the
BRAC Manoshi prject
through reengineering their
healthcare system.
7. Our model provides CHWs with simple JAVA script enabled mobile phones with
built-in medical algorithms. Once patient information has been inserted using mobile
phones, they are digitally sent to the centralized medical information system (MIS) in
real-time.

8. Using mobile phones to collect data can significantly cut the time it takes for a CHW
to fill out a paper form, which can take well over 30 minutes for each household.

comfortably navigate through the medical algorithm on mobile phones within just a couple of hours.
9. ClickDiagnostics’ automated risk assessment algorithm keeps track of pregnant
mothers and assess the level of their risks, enabling CHWs to prioritize their tasks and
visits. A specialist can also access the real-time data from CHWs to monitor conditions
of risky patients and address them in a timely manner.

ClickDiagnostics’ mHealth mechanism has empowered CHWs in several ways.
It increased their work efficiency, minimized mistakes commonly made in manual
recording on paper, and elevated the level of credibility CHWs receive from patients
as an effective medium of health services.
