Organizational Change Strategies for

Evidence-based practice, a crucial competency for
healthcare providers and a basic force in Magnet
hospitals, results in better patient outcomes. The
authors describe the strategic approach to support
the maturation of The Johns Hopkins Nursing
evidence-based practice model through providing
leadership, setting expectations, establishing struc-
ture, building skills, and allocating human and
material resources as well as incorporating the
model and tools into undergraduate and graduate
education at the affiliated university.
Evidence-based practice (EBP) is an essential com-
ponent of professional nursing,1,2 a crucial compe-
tency for healthcare providers,3 and a basic force in
Magnet hospitals4 and results in better patient out-
comes and higher levels of nursing autonomy.5
Fostering EBP within organizations requires strong
infrastructure, including nursing leadership and hu-
man and material resources.6-10 Several organizations
have reported on the use of EBP change models to
assist and mentor individual EBP project teams.11-14
One recent publication discusses the use of a change
model in the context of organizational change,
highlighting the establishment of an EBP committee
that is positioned within the nursing department’s
administrative structure.15 Approaching the imple-
mentation of EBP as an organizational transforma-
tional change frames the approach strategically.16
After the creation and testing of a conceptual
model for EBP,17 a strategic plan was developed to
implement the Johns Hopkins Nursing EBP model
and guidelines (JHN EBP) throughout the organi-
zation. The team knew that the implementation of
EBP would require a substantial change in nursing
culture. The goal was to infuse the use of JHN EBP
into routine practice within each department. This
goal required a number of strategic objectives that
included developing EBP education programs and
Web-based resources, modifying job description cri-
teria to include behavioral outcomes for EBP, defin-
ing the origin of potential question generation, and
building nurse EBP skills and expertise (Table 1).

Higher educational system has gone through substantial reforms and changes
vis-à-vis curriculum innovation over the past years. The evaluation of a revised program is one of the most relevant courses of action done when curriculum change is
to be implemented. One of the main reasons is that it is a chance for practitioners
to test for themselves if their plan is working. It also serves as an identification of
the strengths and weaknesses of the said intervention. This study was conducted to
assess the results of the implementation of the Bachelor of Science in Medical Labo
-ratory Science program. It employed descriptive survey using two types of self-made
questionnaire, the Likert scale and open-ended type survey. Likewise, data on the
results of the graduates’ licensure examinations and status of employment were also
analyzed and correlated. Eighty two graduates from 2010 to 2012 of the enhanced
Medical Technology program of LPU-Batangas and 13 chief medical technologists
from identified affiliate-hospitals were surveyed about their perception on the at
-
tainment of the objectives, the realization of the core competencies as well as the
supposed strengths and weaknesses of the program. Results showed that objectives
were achieved alongside the significant improvements in the board performance and
employment rate were noted. This study can be used as a pilot study for other higher
education institutions with the same health program. This can also be used as a basis
for a curricular reform by assessing the different parameters that were identified.

Since 2002 there have been 1342 residents who have taken the POA, 1255 of those are interns. POA Content: 1. Focuses on knowledge and skills needed during the first 6-18 weeks of their residency. 2. Emphasizes clinical situations that are often encountered without formal supervision. METHODS: - Assessments include: hand-off, informed consent, geriatric functional assessment (or pediatric history taking), aseptic technique, evidence-based medicine, diagnostic images, critical laboratory values, order writing, cross-cultural communication, surgical fire safety, and pain assessment. RESULTS: - The POA Overall Score represents a moderate, statistically significant predictor of future performance. CONCLUSIONS: 1. Hospital orientation proves to be an ideal time to gather initial performance data on entering post-graduate trainees. 2. Baseline performance on the ACGME general competencies maybe a predictor of future board performance. NEXT STEPS: - Many institutions/ program have adopted an intern assessment. - Further work is needed on the predictive value of the POA. - Curriculum efforts must be adjusted to reflect intern deficits.
