INTRODUCTION
The purpose of this training is to provide a working tool which assists
HSE Supervisor to:
– Comply with HSE duties and responsibilities.
– Ensure HSE Management is in accordance with
HSE Policy of the Project.
– Improve their safety performance.
– Reporting safety observation and made
correction of unsafe acts and conditions.
– Be motivated and motivate their subordinates
in safety awareness.

• Occupational Health & Safety?
Conditions and factors that affect, or could affect the health and safety of employees
or other workers (including subcontractor personnel), visitors, or any other person in
the workplace
• Environment?
The surroundings and conditions in which a company operates or
which it may affect, including living systems (human and other)
therein

The potential to cause harm, including ill health and injury, damage to

• Incident?
Work-related event(s) in which an injury or ill health (regardless of severity) or fatality
occurred, or could have occurred.
o An accident is an incident which has given rise to injury, ill health or
fatality.
o An incident where no injury, ill health or fatality occurs may also be
referred to as a near-miss, “close call” or “dangerous occurrence”.

An undesired event causes injury, damage or harm to man, material or environment.

Combination of the likelihood of an occurrence of a hazardous event or exposure(s)

Five steps to correct an employee ?
o Specifically IDENTIFY the unsafe act or condition you see and
want corrected .

change in behaviour.

• Objectives of HSE Inspections:
o Keep a continuous control of the working site status.
o Identify unsafe situations.
o Monitor deviations from safety standards.
o Recommend corrective and preventive measures.
o Ensure safety and environmental recommendations have been implemented.
• HSE Supervisor performs two kind of Inspections:
Informal: daily-routine basis and allow the supervisor to take corrective actions
straightaway.
Formal: specifically scheduled in a weekly or monthly basis according to the
target of the inspection.

o A safety inspection is a thorough examination of the
workplace to ensure a safe work environment.
o Its purpose is to detect existing and potential hazards, which
can be corrected before an incident occurs. Safety
inspections can happen as often as every shift; or they can be
scheduled daily, weekly, monthly, or on some other regular
basis.
o A safety inspection program is a key element to a successful
safety management system. Conducting regular safety
inspections should be a part of every Supervisor’s
performance evaluation.

Continuous inspections are generally done by supervisors
and foreman each time they pass through their area of
responsibility.
Supervisor is accountable for the safety of the workers
under their control; therefore they should be constantly
on the lookout for any hazards that might arise in the
work areas.
Supervisor should ensure that workers are carrying out
preoperational checks when and where they required.

What must be inspected in the work place?
Safety hazards; e.g., inadequate machine guards, unsafe workplace
conditions, unsafe work practices, awkward postures arising from
improper work methods and improperly designed workstations, tools,
and equipment.
Chemical hazards caused by a solid, liquid, vapor, gas, dust, fume or mist.
Physical hazards caused by noise, vibration, energy, weather, heat, cold,
electricity, radiation and pressure.
How often?
Inspection must be done at intervals that will prevent the development
of unsafe working conditions.

Hazard is something with the potential to cause harm to people,
damage equipment or environment.
Risk is the likelihood that the harm will occur from exposure to the
hazard.
For example:
The hazard is electricity. The risk is the likelihood that a worker might be
electrocuted because of exposure to electrical wires that is inadequately
insulated.
The hazard is a 40 kg bag. The risk is the likelihood that a worker might suffer
back strain from manually lifting 40 kg bags.
The hazard is carbon monoxide. The risk is the likelihood that a worker might
suffer carbon monoxide poisoning because they are using a petrol operated
pump in a well.

Risk assessment is nothing more than a careful examination of
what in your work, could cause harm to people, so that you can
weigh up whether you have taken sufficient precautions or
should do more to prevent harm.

The important thing you need to decide that
whether a hazard is significant and whether
you have it covered by satisfactory precautions
so that the risk is small.
For instance Electricity can kill but the risk of it
doing so in an office environment is remote,
provided that live components are insulated and
metal casings properly earthed.

Task Risk Assessment is a technique that focuses on job tasks
as a way to identify hazards before they occur. It focuses on
the relationship between the worker, the task, the tools, and
the work environment.
Ideally, after you identify uncontrolled hazards, you will take
steps to eliminate or reduce them to an acceptable risk level.
Task Risk Assessment is therefore a fundamental part of you
activities.

Taking Shortcuts: Every day we make decisions we hope will make
the job faster and more efficient. But do time savers ever risk your
own safety, or that of others? Short cuts that reduce your safety on
the job are not shortcuts, but an increased chance for injury.
Being Over Confident: Confidence is a good thing. Overconfidence is
too much of a good thing. "It'll never happen to me" is an attitude
that can lead to improper procedures, tools, or methods in your
work. Any of these can lead to an injury.

Poor Housekeeping: When visitors, managers or safety professionals
walk through your work site, housekeeping is an accurate indicator of
everyone's attitude about quality, production and safety. Poor
housekeeping creates hazards of all types. A well maintained area sets
a standard for others to follow. Good housekeeping involves both pride
and safety.
Ignoring Safety Procedures: Purposely failing to observe safety
procedures can endanger you and your co-workers. You are being paid to
follow the safety policies - not to make your own rules. Being "casual"
about safety can lead to a casualty!

Failure To Pre-Plan The Work: There is a lot of talk today about Risk
Management. RM is an effective way to figure out the smartest ways
to work safely and effectively. Being hasty in starting a task, or not
thinking through the process can put you in harms way. Instead, Plan
Your Work and then Work Your Plan!
Mental Distractions From Work: Having a bad day at home and
worrying about it at work is a hazardous combination. Dropping your
'mental' guard can pull your focus away from safe work procedures.
You can also be distracted when you're busy working and a friend
comes by to talk while you are trying to work. Don't become a
statistic because you took your eyes off the machine "just for a
minute."

Conditions and factors that affect, or could affect the health and safety of employees or other workers (including subcontractor personnel), visitors, or any other person in the workplace • Environment? The surroundings and conditions in which a company operates or which it may affect, including living systems (human and other) therein • Hazard? CONFIDENTIAL Page 42 HSE INSPECTIONS • Objectives of HSE Inspections: o Keep a continuous control of the working site status. o Identify unsafe situations. o Monitor deviations from safety standards. o Recommend corrective and preventive measures. o Ensure safety and environmental recommendations have been implemented. • HSE Supervisor performs two kind of Inspections: Informal: daily-routine basis and allow the supervisor to take corrective actions straightaway. Formal: specifically scheduled in a weekly or monthly basis according to the target of the inspection. HSE for Supervision
