This document discusses using IT systems to support population health and patient-provider interactions. It describes two aspects of population health systems: systems that facilitate patient-provider interactions, and systems that communicate, aggregate, and report population health information. It then focuses on examples of clinical electronic decision support systems and communication support systems currently in development or use for various health conditions.
