Pilonidal sinus disease is a soft tissue infection. It presents either acutely with abscess or the chronic form of sinus formation. The disease affects multiple body regions but the commonest is the sacrococcygeal region. There are different treatment strategies ranging from simple incision and
drainage of an abscess to complex constructive procedures.
BACKGROUND: Penetrating carotid artery injuries (PCAI) in civil time are infrequent, yet they present significant diagnostic and therapeutic challenges and can be associated with significant morbidity and mortality. Proper resuscitation and urgent exploration is necessary for actively bleeding patients.
OBJECTIVE: The aim of this paper is to present our humble experience in management of such injuries with literature review.
PATIENTS AND METHODS: Herein, we present 5 cases of penetrating carotid artery injuries managed in Sulaimania and Basrah from January 1996 to 30th of November 2009.
RESULTS: All patients were young males. Three injuries were located in zone III and 2 in zone II. Four patients presented hours to days after the injury while the fifth presented after few months. Angiography was done in 2 patients with a false aneurysm of internal carotid artery (ICA). All 3 patients with ICA injuries were managed by ligation due to profuse bleeding and poor access. The 2 patients with common carotid artery (CCA) injuries in zone II had an end to end repair. All 5 patients have survived without significant neurological deficits.
CONCLUSIONS: Penetrating carotid artery injuries in zone II usually do not require preoperative angiography unlike those in zone I and III. Repair is always desired. It is a straightforward operation for zone II injuries but really challenging for zone III due to poor access. Certain zone III injuries may be just observed or treated by endovascular stenting when facilities permit. Ligation of ICA carries a high risk of stroke; however, young people with well developed circle of Willis may tolerate it well.
Key Words: carotid artery, penetrating injury, zone I, II and III, neurological deficit.

Diagnosis of ruptured pulmonary hydatid cyst by means of flexible fiberoptic ...Abdulsalam Taha
There are three radiological signs considered diagnostic of ruptured pulmonary hydatid cyst (PHC): perivesicular pneumocyst, double domed arch and water lily. Apart from these, every localized radiological density seen in any patient above the age of 3 in an endemic area should be looked upon as possible ruptured hydatid. Nevertheless, situations where the diagnosis of ruptured PHC is difficult are far from being rare in countries of high endemiology. Thus a preliminary bronchoscopy is a perfectly justifiable step in the diagnostic work-up. Herein, we report 3 selected cases of Iraqi patients with ruptured PHC in whom definitive diagnoses were made using the flexible fiberoptic bronchoscope (FOB).

Foreign body aspiration in healthy adults is uncommon, happening mainly accidentally. Despite few need medical assistance, there are cases in which the object is lodged in areas of the upper airways and specialized care is needed, namely in the removal of the item. Case Presentation: We report a case of a snail aspiration with consequent lung atelectasis and need of quick intervention. The foreign object remained in the right mainstem brochi and was removed by bronchofibroscopy, with successful recovery afterwards.
Unusual Presentation of Hydatid Cyst in a Childsemualkaira
Pulmonary hydatid disease remains an important healthcare problem in children. We report a case report of a 4-year-old child with chronic cough and hemoptysis for 2 months. he had no contact with dogs. Pleuropulmonary examination showed well-transmitted bladder murmurs and well-perceived vocal vibrations. The diagnosis of pleuropneumopathy was retained and the patient was treated with amoxicillin - acid clavulanic antibiotic therapy. In front of non-clinical improvement, ultrasound showed a left basal, rounded, thickened-walled, non-vascularized, non-doppler, single loculate basal cyst formation which is related to a hydatic cyst . Surgey consisted of a left posterolateral thoracotomy associated with a puncture-evacuation of the contents of the hydatic cyst, an exterioration of the proligeus membrane. The recovery was good.

Histoplasmosis is a systemic mycosis present in an endemic state in many tropical or temperate regions. It is a pathology induced by Histoplasma capsulatum, dimorphic fungus thriving in wetlands and at moderate temperatures. In immunocompetent subjects, 95% of clinical forms are asymptomatic. The preferred sites for this dissemination are pulmonary, medullary, hepatic, splenic and ganglionic. Exceptionally, the fungus can reach the eye.

2. The Case
A young adult male patient was
admitted to Sulaymaniyah Teaching
Hospital, Sulaymaniyah, Region of
Kurdistan, Iraq on the 25th of
September 2006 because of shortness
of breath (SOB) and bouts of dry cough
of a few months duration. CXR revealed
a big rounded smooth-outlined
homogenous opacity in the left upper
pulmonary zone. 2
5. The Case…
The patient was provisionally
diagnosed as a case of intact
pulmonary hydatid cyst as he had the
typical radiographic appearance of
(Full moon against dark sky). He was
subjected to left posterolateral
thoracotomy and the cyst was
removed by aspiration and
evacuation technique. The
postoperative course was smooth.

6. Comment
Hydatid disease is a dynamic entity
with varying imaging appearances.
Familiarity with imaging findings,
especially in patients living in
countries where this disease is
endemic, provides important
advantages in making the diagnosis.
Hydatid cyst should be kept in mind
when a cystic lesion is encountered
anywhere in the body [1]. 6
