Unusual Presentation of Hydatid Cyst in a Childsemualkaira
Pulmonary hydatid disease remains an important healthcare problem in children. We report a case report of a 4-year-old child with chronic cough and hemoptysis for 2 months. he had no contact with dogs. Pleuropulmonary examination showed well-transmitted bladder murmurs and well-perceived vocal vibrations. The diagnosis of pleuropneumopathy was retained and the patient was treated with amoxicillin - acid clavulanic antibiotic therapy. In front of non-clinical improvement, ultrasound showed a left basal, rounded, thickened-walled, non-vascularized, non-doppler, single loculate basal cyst formation which is related to a hydatic cyst . Surgey consisted of a left posterolateral thoracotomy associated with a puncture-evacuation of the contents of the hydatic cyst, an exterioration of the proligeus membrane. The recovery was good.

There are three radiological signs considered diagnostic of ruptured pulmonary hydatid cyst (PHC): perivesicular pneumocyst, double domed arch and water lily. Apart from these, every localized radiological density seen in any patient above the age of 3 in an endemic area should be looked upon as possible ruptured hydatid. Nevertheless, situations where the diagnosis of ruptured PHC is difficult are far from being rare in countries of high endemiology. Thus a preliminary bronchoscopy is a perfectly justifiable step in the diagnostic work-up. Herein, we report 3 selected cases of Iraqi patients with ruptured PHC in whom definitive diagnoses were made using the flexible fiberoptic bronchoscope (FOB).

Prevalence of massive obesity continues to increase and only
bariatric surgery has succeeded in providing sustained weight
loss[1]. Laparoscopic Sleeve Gastrectomy (LSG) accounts for approximately 30% of bariatric procedures performed worldwide, and
its coexistence with situsinversus is one in a million. Most of the
global documented procedures in situsinversus involve gas-tric
bands, gastric ypass, and cholecystectomies [2]. Situs Inversus is a
congenital developmental anomaly wherein the abdominal organs
are reversed or mirrored to the opposite side of the body through
the sagittal plane. In contrast, situsinversus totalis is a similar
condition in which both the thoracic and abdominal con-tents are
reversed [2]. Transmitted through an autosomal recessive
inheritance, these anomalies have been in vogue since the 17th
century

The occurrence of a broncho-pleural (BPF) after pneumonectomy is an infrequent but severe complication accompanied by a high morbidity and mortality. Small BPFs may heal either spontaneously or with drainage only. However, the majority of patients with persistent BPFs require operative intervention. There is no standard treatment to this complication and the successful management is a challenge to the thoracic surgeon. While most of the treatment options are staged operations, the trans-sternal trans-pericardial (TSTP) closure is attractive as it is a one stage operation that avoids the infected pneumonectomy space and does not result in patient’s disfigurement. The technique was first used in Italy and then used extensively in the former Soviet Union. Herein, we report a case of chronic BPF after pneumoectomy successfully closed via the TSTP approach. The relevant literature is reviewed to throw light on the indications and the results of this operation.

2. The Case
A 7-years old boy was admitted to
Sulaymaniyah Teaching Hospital,
Sulaymaniyah, Region of Kurdistan,
Iraq on 4th of Feb 2010 because of
anorexia and weight loss of one year
duration. On examination, he was
pale and small for his age. Chest
examination revealed bilateral
wheeze.

4. The chest radiograph showed a huge rounded
opacity occupying right upper and middle zones
with small ill defined opacities in the left middle
zone.

5. Ct scan of chest and abdomen displayed an intact cyst
in right lung, multiple small ruptured and intact cysts in
left lower lobe and a small cyst in the liver.

11. The Case…
The patient was prepared for surgery on the
right chest. Right posterolateral 5th space
thoracotomy was done. A huge intact cyst
completely destroying the RUL with adhesions
to chest wall was found. The cyst was
surrounded by packs soaked in Povidone iodine.
The cyst’s contents were removed by aspiration
and evacuation method. Then right upper
lobectomy was performed in the routine way.
Hemostasis and aerostasis were secured. Two
chest drains were placed. The chest wall was
closed in layers. The postoperative course was
smooth. 11
16. Comment
This was a case of multiple bilateral
pulmonary and liver hydatid cysts in a
child. Multiple pulmonary hydatid cyst is
not rare in areas in which hydatid disease
is highly endemic [1].
Although the lung is next to liver in
involvement with hydatid disease, it is
operated upon first when both organs are
involved. Untreated PHC may rupture
during surgery and threatens the life of
the patient. 16
17. Comment…
If bilateral intact PHC are
encountered, surgery is offered for the
bigger cyst first. However, if one cyst
is already ruptured, surgery is done on
the intact cyst first to avoid the
complications of its sudden rupture.
Lobectomy was necessary in this case
due to the irreversible damage of the
RUL produced by the huge size of the
cyst.
