2. The Case
A 24-years old man was admitted to
Sulaymaniyah Teaching Hospital,
Sulaymaniyah, Iraq on the 6th of July
2009 because of a stab wound close to
the suprasternal notch on the right side
(Zone I neck injury). He was clinically
stable with no evidence of external or
internal bleeding or intra-thoracic injury.
However, angiography was planned for
exclusion of major vascular injuries.

6. Comment
The neck is divided into three zones. Zone
I, including the thoracic inlet, up to the
level of the cricothyroid membrane, is
treated as an upper thoracic injury. Zone
III, above the angle of the mandible, is
treated as a head injury. Zone II, between
zones I and III, is the area of controversy.
Zone I extends between the clavicles and
the cricoid cartilage; injuries to this zone
carry the highest mortality because of
vascular injury and high-risk surgical
explorations.
