3. All innovation projects shall be submitted to the School Management Monitoring & Evaluation (SMME)
Unit for processing, evaluation and validation.
4.
All innovation project proposals to be conducted by school heads and Division Office Personnel shall be
forwarded directly to the Division in-charge for processing, evaluation, and validation, following the
prescribed templates.
5. All innovation project proposals to be conducted by school teaching and non-teaching personnel shall be
indorsed by the School Evaluation Committee, following the prescribed templates.
6. In case of a group, the innovation project shall consist of a maximum of 3 members. Function of each member
shall be identified.
7. Innovation projects proposals shall be submitted to the SMME Unit thirty (30) days before the
implementation.
8. A Certificate of Completion shall be given to proponent/s upon completion of the innovation project.
9. Attached are the following enclosures to guide the proponent/s in conducting the innovation project:
a. Revised Guidelines on the Conduct of Innovation Projects under School Governance and
Operations
b. Template – Format in Writing Innovation Project Proposal
c. Template – Format in Completed Innovation Project
d. Template – Innovation Project Completion Report
e. Template – Approval Sheet (School & Division)
f. Template – Indorsement for the Innovation Project Completion Report
g. Template – Indorsement for the Permit to Conduct Innovation Project
h. Template – Permit to Conduct Innovation Project (School & Division)
10. Soft copy of the templates can be downloaded at tinyurl.com/pang2IPPSGODunit using the DepEd email.
11. All on-going innovation project proposals shall abide to the new template for the submission of the
completion report.
12. Immediate and wide dissemination of this Memorandum is desired.
