Viral fevers like H1N1 and dengue are resulting in high hospitalization costs in India, with average treatment claims being between Rs. 50,000 to Rs. 3 lakh over 5 days. Hospital bills for patients requiring intensive care and ventilator support can exceed Rs. 10-14 lakh. Insurance claims for fever-related infections have risen 7% year-over-year in Mumbai. Experts warn that infectious disease burden in India may rise to the level of cardiac diseases due to increasing pollution, poor lifestyles and immunity. However, some note that hospital bills are often inflated due to unnecessary tests and procedures.

When Rajiv Shah's five-year-old daughter was diagnosed with H1N1 last week, he
had to shell out Rs 60,000 for four days of hospitalization. “She was kept in isolation
so that other patients weren't exposed to the virus,“ said Shah (name
changed).Elsewhere in the suburbs, an H1N1 patient has been on ventilator
support for the last 40 days and administered some of the most potent antibiotics
along with anti-virals.
“At a very conservative estimate, the patient's bill must be between Rs 10 and Rs 14
lakh so far,“ said a doctor treating him. Viral fevers are not only debilitating and
proving fatal at times, they are now also resulting in hospitals bills worth lakhs. “If
an H1N1 influenza patient needs ICU and ventilator support, apart from isolation,
the daily bill can climb to Rs 35,000,“ said a senior doctor. She added the bill could
increase by another couple of lakhs if a patient needed specialized tech niques, like
extracorporeal membrane oxygenation, to fight the infection. Hospitals also levy
isolation charges and special duty allowance for nurses who can't then attend to
other patients, in addition to the cost of medication.

In a year when H1N1 has claimed 44 lives in Mumbai and affected over 2,700 Mumbaikars,
data from an insurance company showed claims for fever-related infections (read viral
fevers) increased by 7% in just one year.
“If we compare the number of claims put in for fever and infection in the July-August
period in 2014 with the same period this year, there has been a 7% rise,“ said a
spokesperson for ICICI Lombard. In Delhi, the rise was 2% while Bangalore was the highest
at 8%.
Claims for fever and infections in Mumbai increased by 16% from 2011 to 2014 while Delhi
was even higher at 18%, said Sanjay Dutta from the insurance company . Every third claim
for an infectious disease-related hospitalization concerned fever and infection.Claims for
five-day hospitalization were, on an average, between Rs 50,000 and Rs 3 lakh.
Experts often say India has transformed from a country plagued by infectious diseases to
one with a staggering burden of non-communicable diseases like hypertension and
diabetes. But during the monsoon months, infectious diseases like viral fevers, dengue,
jaundice and typhoid return to the fore.

4. • Dr Om Srivastava, an infectious diseases specialist, said the burden of such
diseases is again rapidly increasing. “The burden may become as high as
that of cardiac diseases because of our lifestyles and pollution,“ he said.
• Since cities have over 80% humidity levels, pathogens from the dirt and
pollution multiply more rapidly. “Moreover, we sleep less, eat wrong stuff,
work very long hours and don't know how to handle stress. Our immunity
is low, thereby increasing the chance of contracting an infection,“ he said.
• Dr Kushrav Bajan, an intensive care specialist, said claims for infectious
diseases are usually higher than those for non-communicable diseases.
“Still, the claim amount for infectious diseases is usually not as high as, say,
a heart operation except when the patient needs ventilator support,'' he
said.

Dr Randeep Guleria, professor of medicine at AIIMS, pointed out that
fever is a nonspecific cause of admission.“It is possible that fever is
shown as cause for admission or claim since it is reimbursable,“ he said.
However, activist Asha Id nani of Consumer Guidance Society of India
said hospital bills are usually inflated due to unnecessary tests and
procedures.
“People fear hospital bills so much that they delay going to hospitals
until they are very serious,“ she said.
