WHAT HAPPENED?
On 14th DEC 2023 around 3.40 PM, Gas cutter have been engaged for Pipe line cutting activity at ‘’L9” After
cutting the angle (approx. 150 mm lengths), Gas cutter try to placed the angle at working platform. While
placing Gas cutter’s hand slipped and angle fell through the gab in the plat form 7 Meter height
(approximate). No injuries were sustained by workmen.
CAUSE FOR THE ACCIDENT:
Loose material stored at the working platform
Gab between planks in the working platform
Ground level cordon off not provided
Inadequate training for gas cutter

PREVENTIVE ACTIONS:
After Cutting the angle, initial securing with the help of binding wire at platform.
Ensure no gag between the planks in working platform
Below level cordon off required with watcher to avoid the unauthorized person entry inside secured area.
Adequate training to be provided to the gas cutter and crew
LESSONS LEARNED:
Trained and competent person only shall be ensured on work
Proper Fall protection system shall be ensured at workplace
Proper supervision shall be ensured
Before commencing work, Detailed work methodology shall be prepared at it should be familiar with workmen
crew by JST and the same shall be implemented at work place

NEAR MISS CASES EXAMPLE - 2
WHAT HAPPENED?
On 20th JAN 2024 around 5.42 PM, Tractor was used for Debris shifting work at WTP to stilt-1 ramp area.
Once tractor was loaded by Bobcat, The driver moved the tractor backwards at the time the tractor trailer
rear wheel roll over the steel mesh (which was kept to secure the drain) slipped and the real wheel fell on
the drain whish was 600MM depth. Immediately P&M team involved and lifted the trailer wheel using
Jackie. No Man are Material got damaged during this incident.
CAUSE FOR THE ACCIDENT:
Lack of Banks man for tractor
Illumination not considerate during evening the activity
Steel mesh not fixed properly

Backfilling the drain where ever the vehicle movement.
Provide adequate illumination
LESSONS LEARNED:
Reflective stickers and Causation sinages board shall be located near the vehicle movement area.
Considering the time period of the work and preparing the work place .
Before commencing work, Detailed work methodology shall be prepared at it should be familiar with
workmen crew by JST and the same shall be implemented at work place.
