Common Failure Modes for Lean Deployment
1.Establishing a Sense of Urgency
• No sense of urgency created – no
accountability
• Absence of senior leadership
• No commitment of leadership to understand
change
• Complacency rules the day
2.Creating a Guiding Coalition
• Champions have poor positional power
• No formal coalition established
• Poor commitment to up-front workshops and
training
• No clear accountability for results
• Guiding deployment with weak steering teams
• Not weeding out people who are resisting
change
3.Developing a Vision and Strategy
• No vision has developed – program is a
training program rather than an improvement
program
• Vision is not linked to strategy
• No sense of urgency to support the vision
• Lean not linked to strong results
• Doing Lean to merely “check a box”
4.Communicating the Change Vision
• No communication plan – Lean becomes a
stealth program
• Communication at upper levels but not at
lower levels
• Leadership is not visible in the commitment
and communication
• Under-communicating (“…but I’ve already said
it a hundred times)

5. Empowering Employees for Broad-based
Action
• No leadership commitment to intense
workshops and training (self or employees)
• Little involvement in project selection
• No project tracking that’s easily visible
• Lean viewed as a nice training program
• Little on-site project support given
•
6. Generating Short-term Wins
• No accountability established
• More than 12 months to achieve reasonable
results
• No formal recognition ceremonies with clear
presence of senior leadership
• No clear financial support to establish business
impact
• Launching too many projects at once
• Providing the first win too slowly.
7.Consolidating Gains and Producing More
Change
• Program stagnates
• Internal experts not developed
• Program results not carefully tracked via metrics
• Lean project seen as extra work that detracts
from day-to-day operations
• Convincing the organization that they are done
when they are not
8.Anchoring the New Approaches in the
Culture
• Business as usual – if Lean disappeared, no one
would notice
• Promotions not linked to Lean activities
• No development of internal resources to
support program
• Few changes in systems and leadership style
• Students consistently show up at training with
poorly defined, small projects

Lean Government Center® – KaiNexus February 19, 2018 Webinar 6 Common Failure Modes for Lean Deployment 1.Establishing a Sense of Urgency • No sense of urgency created – no accountability • Absence of senior leadership • No commitment of leadership to understand change • Complacency rules the day 2.Creating a Guiding Coalition • Champions have poor positional power • No formal coalition established • Poor commitment to up-front workshops and training • No clear accountability for results • Guiding deployment with weak steering teams • Not weeding out people who are resisting change 3.Developing a Vision and Strategy • No vision has developed – program is a training program rather than an improvement program • Vision is not linked to strategy • No sense of urgency to support the vision • Lean not linked to strong results • Doing Lean to merely “check a box” 4.Communicating the Change Vision • No communication plan – Lean becomes a stealth program • Communication at upper levels but not at lower levels • Leadership is not visible in the commitment and communication • Under-communicating (“…but I’ve already said it a hundred times)

Empowering Employees for Broad-based Action • No leadership commitment to intense workshops and training (self or employees) • Little involvement in project selection • No project tracking that’s easily visible • Lean viewed as a nice training program • Little on-site project support given • 6. Generating Short-term Wins • No accountability established • More than 12 months to achieve reasonable results • No formal recognition ceremonies with clear presence of senior leadership • No clear financial support to establish business impact • Launching too many projects at once • Providing the first win too slowly. 7.Consolidating Gains and Producing More Change • Program stagnates • Internal experts not developed • Program results not carefully tracked via metrics • Lean project seen as extra work that detracts from day-to-day operations • Convincing the organization that they are done when they are not 8.Anchoring the New Approaches in the Culture • Business as usual – if Lean disappeared, no one would notice • Promotions not linked to Lean activities • No development of internal resources to support program • Few changes in systems and leadership style • Students consistently show up at training with poorly defined, small projects
