разработали эпидемиологическую
классификация гельминтозов, в
основу которой положены
три важнейших признака:
1) наличие или отсутствие
промежуточного хозяина;
2) отношение ранних стадий
гельминтов к внешней среде;
3) механизм
заражения окончательного
хозяина.
Классификация гельминтозов
4. Геогельминтозы – заболевания, возбудители которых проходят часть
своего развития в организме человека, а другую часть – на каком-либо
неживом субстрате (чаще всего в почве) и полное развитие паразита
происходит без участия промежуточных хозяев. К геогельминтозам
относятся: аскаридоз, трихоцефалез, анкилостомидозыи др.
Биогельминтозы – заболевания, возбудители которых часть своего
развития проходят в организме человека, а другую часть – в организме
одного или нескольких промежуточных хозяев, прежде, чем
приобретают способность инвазировать другого человека. К
биогельминтозам относятся: тениаринхоз,т
онхоцеркоз и др.
Контагиозные (контактные) гельминтозы – заболевания, при
которых из организма человека выделяются зрелые или почти зрелые
стадии паразита, способные заражать другого человека. К
контагиозным гельминтозам относятся энтеробиоз и гименолепидоз.

9. Стадии заболевания
Острая - 1-2 мес. (в основном явления аллергизации)
- лихорадка, отеки, высыпания на коже, миалгии,
артралгии, лимфоаденопатия, легочный,
абдоминальный синдромы, гепатоспленомегалия,
эозинофилия в крови, диспротеинемия
Латентная (до начала кладки яиц, Н., аскариды - до 2
мес.)
Хроническая - месяцы и годы (с момента кладки
яиц) - характер развивающихся нарушений и
связанных с ними клинических проявлений в
значительной степени определяется локализацией
возбудителя, его численностью, особенностями
питания.

18. В 2005 году Weinstock et al. провели исследование, включив
в него 52 больных язвенным колитом, 29 из которых в
течение трех месяцев раз в 2 недели принимали перорально
2500 яиц свиного власоглава, а 23 пациента - плацебо.
У 13 пациентов из 29 (44.8%), получавших яйца
гельминтов, было достигнуто клиническое улучшение,
для сравнения в группе плацебо — лишь у 4 из 23
пациентов (17.4%).

Отбор проб кала
• Кал после дефекации отбирают из разных участков в количестве не
менее 50 г.
• Пробу помещают в чистую сухую, стеклянную или пластмассовую
посуду с крышками.
• Проба кала доставляется в лабораторию и исследуется в день
дефекации.
• При невозможности исследования пробы кала сразу после дефекации
или в день поступления материала в лабораторию, пробу кала хранят при
температуре от 0 до 4 °С не более суток или собирают в консервант
• Пациенту рекомендуется не употреблять в пищу грибы, печень,
большое количество грубой клетчатки, принимать сорбенты.
• После масляных клизм и приема бария должно пройти несколько суток.
• В случае лечения антибиотиками широкого спектра действия или
антибактериальными препаратами кал для исследования следует начать
собирать спустя 7-10 дней после окончания приема препаратов

Time of admission: 23 of January 2012
Complains at admission: a long history of slightly
painful erythema at a left region of a small of the back,
which had started to spread up
to the left scapula 3 days ago.
Anamnesis morbi: at 9 of June
2011 patient visited her friend'
summer cottage situated in a
suburb of St-Petersburg, Russia.
She was swimming in a outdoor

suddenly she felt a sharp pain
in the left side of the small of
her back.

Epidemiological data: patient visited Kazakhstan
(country in Central Asia) in May 2011, but didnt
remember any significant health problem there
Anamnesis morbi: patient have obesity, moderate arterial
hypertension and myoma
She is working for private company and live in a city'
appartment with a good sanitary conditions
Patient friends who invite here to swim in a pool have a
dog which live in a kennel outside house
Patient have 2 cats which are not allowed
to go outside house and have regular
veterinary check-up
What is a diagnosis?

Surgeon diagnosed erysipelas and adviced patient to
continue amoxiclav for 5 days
Since there was no improovment patient visited another
surgen who said he have doubts about erysipelas and advice
to make ultrasound examination and to change treatment.
Ultrasound of the affected area revealed nothing significant.
Compress with antiseptic and physiotherapy were prescribed
for 10 days with positive effect.
After disapperaing of the edema and redness patient was still
able to feel a small nodule in a skin but surface of a skin
wasnt affected so surgeon didnt pay attention to it.
10 days later all symtoms were coming back. Patient visited
same surgeon who reffered her to consultation with
dermatologist.
Case 5: 46 y.o. women with erythema

Patient was examined by professor of dermatology who
diagnosed erythema migrants (Lyme disease) and prescribed
serological testing
Serology for JgM/JgG to Borrelia burgdorferi come negative
which was explained by doctor as a effect of previous antibiotic
treatment (amoxiclav), patient was prescribed doxycycline 0,1
bid for 28 days and local ichthyol applications.
After 7 days on treatment sympoms went away and patient
finished prescribed course of doxycycline at 26 of December
2010.
02 of January 2011 edema and redness started again

Patients visited her dermatologist only at 12 of January 2012.
By that time edema and redness became less prominent
without any interventions.
A dermatologist had an consilium were collegue of professor
suggest that it might be «something» inside. Patient was very
scarred and decided to visit infection disease specialist.
She was examined in outpatient clinic in Botkin's infection
disease hospital at 16 of January 2012.
Ultrasound — a 4 cm subcutaneous consolidation were
revieled with 3 fluid inclusions (1 cm each) inside it.
Blood count: WBC 11,6; moderate bound shift to the left;
Eos 3% (N less then 5%); ESR 26 mm/hour
Blood JgE 156 IU/ml (N 20-100 IU/ml)

10 cm long germ was found in subdermal tissue in a wound
Germ was examinned in a city parasitological laboratory and
was found to be a Dirofilaria repens.
After healing of a wound patient was discharged from a
hospital.
Since patient had visited Kazachstan a month before symptoms
started we cant exclude a possability that this case was
imported from there. No local cases of D. repens were reported
previously in St-Petersburg and it suburbs.

52. Case 5: Dirofilariosis
-Dirofilariosis is a filarial nematode of dogs and other
carnivores (cat, wolves et cetera)
-2 main types are D. repens and D. immitis
-Mature filaria (macrofilaria) shed microfilaria into animal
blood
-Microfilaria could cause damage of pulmonary vessels and
heart in animals
-Microfilaria is transmitted by mosquitos which can feed on
both animals and humans (Aedes, Culex Anopheles).и
-Although humans may become infected as aberrant hosts,
the worms fail to reach adulthood while residing in a human
body (there is some report when microfilaria was found in
human blood as well)
-During 1995-2000 372 new cases were diagnosed in 25
countries around the world, at the end of 2003 -782 in 37
countries.

-In humans D. repens in most cases are found subcutaneously.
-Ophtalmological manifestations are most common.
-South and Western Europe, Central Asia, Shri-Lanka are
endemic. More cases from south and central part of Russia
recently (around 200-300 cases totally)

According to study conducted in Russia when medical notes of
114 patients with dirofilariosis were examined there were no cases
when helmintosis were suspected during first visit. Patients visited
different specialsts (surgeon, general practicioner, neurologist)
and in some cases a followed diagnosis were made:
- Ateroma of the soft tissues of the forehead
- Postvacinal reaction
- Neurofibroma of the soft tissues of left shin
- Cancer of the chest wall
- Reactive lymphoadenopathy
- Idiopathic dilatation of the blood vessels of the right eye

• Уртикарная сыпь (по типу крапивницы), особенно в
сочетании с эозинофилией
• Wells syndrome эозинофильное гранулематозное
воспаление подкожной клетчатки
• Хроническая крапивница (?) (у 65% пациентов с
хронической крапивницей (n=51) в крови выявляли
антитела к токсокрам, в контроле - только у 21%
(n=81). В другом исследовании разница составила 20%
(n=128) и 13% (n=236) соответственно)

73. Case 6: 31 y.o. with haemorrhages in scleras
Time of admission: 15 of April 2012
Complains at admission: high fever, moderate pain in eyes,
residual changes in sclearaes of both eyes (minimal
haemorraghes around iris).
Anamnesis morbi: at 8 of April 2012 patient noticed fever
and pain in eyes which was accompanied by moderate
periorbital edema and
haemorraghes in sclerae
of both eyes. At 9 of April
patient visited ophtalmologist
who prescribed himt an eye
drops with vitamins and told
him that his complains were
related to stress and adviced
him to visit neurologist.
74. • Same day patient had visited neurologist who
recommended him to make MRI of a brain. MRI revealed
no significant changes.
At the evening of 9 of April fever had reached 40C,
patient took 1 gr of paracetamol and call ambulance. By
the time doctor arrived fever decrease to 37C, doctor from
ambulance service told patient he never saw such
significant haemorrages in scleares and recommended
patient to visit infection disease spaecialst (IDS).
Patient visited IDS at 10 of April with complains on
haemorrages in sclera, moderate muscle pain and fever.
Fever raised up to 38-39C and responded to antipyretics
for a short time.

IDS ask patient to do a number of tests including tests for
herpes simplex infection of the eyes, but patient refused to
do tests and went home.
For 3 days he stayed at home with high fever (patient took
paracetamol with timely effect), haemorraghes in scleares
gradually decreased.
At 5th day of disease patient visit GP and give blood for
general blood count. No changes in treatment was made by
GP.
Despite of a fever next day he was able to visit his friend
wedding ceremony where he felt generally weak and didnt
have appetite.

Epidemiological data: 10 days before patient got ill he
went for a barbique with his wife, daughter and his
friend with bribe. For barbeque they had bought a fresh
pork meat in one of the city' supermarket.
2 days after barbeque patient' wife had diarrhea for 2
days.
7 days later patient friend developed high fever,
moderate muscle pain and periorbital edema.

Physical examination: patient looks generally well,
179cm/82kg, skin was clear.
Blood pressure 120/70 mmHg, pulse 83 per minute, arythmic
with few extrasislols
Lungs were clear. No dyspnea.
Liver and spleen was normal in size. Palpation of abdomen
was unpainful.
24 hours urine volume and urine color were normal.
Meningeal symptoms were negative.
Palpation of muscles was unpainful.

88. Paget's disease of bone is a chronic disorder that can result in
enlarged and misshapen bones. The excessive breakdown and
formation of bone tissue causes affected bone to weaken, resulting in
pain, misshapen bones, fractures, and arthritis in the joints near the
affected bones.
89. - Patient' serology was higly positive for
antibody against Trichinella spiralis. Later
his friend who made barbeque with him
( look at epidemiology slide) had same
result.

worm and 5 species of Trichinella described
they are distributed among carnivorous on
all continents
- Adult: Female 4 mm*0,06 mm
Male 1,5mm*0,03mm
- Adult live in small intestine for around
2 months, lavrae can survive in striated
muscles for many years

of larvaes
• After reaching bloodflow larvae
spreaded through different tissues
specifically striated muscles (but
also heart, lungs, brain) and
entering muscle cell where
forming a cyst
• Larvae stimulate angiogenesis in
the area around it and causing
formation of a calcification
93. - GI phase usually developed in 12-48
hours after eating infected meat and
accompaning by nausea, vomiting,
diarrhea and abdominal discomfort.
- Within a 3 weeks later due to spreading
of a lavras from small intestine through
bloodflow and lymphatic vessels patient
might develope fever, fascial edema,
hemorrhages in sclerae, MUSCLE
PAIN, heart failure, respiratory
symptoms and meningoencephalitis.
- There is usual eventual complete
recovery in most patients but mascular
pain (60%), fatigue (25%) and heart pain
may persist for 1-6 years.

epidemiological data (eating a
undercooked meat, especially pork, boar
or bear),
high eosinophilia (up to 90%),
serological tests (ELISA)
muscle biopsy (usually from biceps)
- Treatment: 10 days of albendazole (400 mg
bid) or mebendazole (200 mg bid) or
thiabendazole (25 mgkg bid) with
corticosteroids.
- Prevention: trichinelloscopy of meat,
hygienic pig farming, apropriate
cooking of meat

• peripheral eosinophilia count (> 3 × 109/L);
• elevated serum IgE levels (> 1000 kU/L);
• increased titres of antifilarial antibodies;
• peripheral blood negative for microfilariae; and
• clinical response to diethylcarbamazine.

100. 54 летний российский бизнесмен владеет собственным рестораном на
севере Китая и большую часть времени проводит там. У него есть
небольшое ожирение и умер артериальная гипертензия, которую он не
лечит
101. 3 месяца назад у него появилась одышка рпи физической нагрузке, одышка постепенно
нарастала, через месяц он решил обратиться к врачу. В китайской больнице со слов
больного сделали клин анализ крови и рентген - в норме. Пациент обратился за
помощью к традиционной китайской медицине - принимал травы и делал
иглоукалывание - без эффекта
102. 27 декабря одышка еще более усилилась, появился кашель с мокротой и кровью, лихорадка
до 38-39С, отеки ног, пациент прилетел в СПб и был госпитализирован в инфекционный
стационар. Проведена плевральная пункция - эвакуировано 1600 мл геморрагической
жидкости, после чего одышка несколько уменьшилась, в этот же день выполнен контр
рентген. 7 лет назад пациент провел неделю на острове в Желтом море, где у его друга
ферма по разведению крабов и улиток - наш пациент ел там множество этих созданий в
полусыром виде, однако отрицает респираторные жалобы до данного заболевания.

• Ранняя миграционная фаза болезни может проявляться симптомами
энтерита, асептического перитонита, гепатита (2-3 недели)
• Через 2-3 месяца - симптомы поражения легких, по ВОЗ:
1. Кашель более 3-х недель;
2. Кровохарканье rusty;
3. Пациенты с клиническими или рентгенологическими признаками
туберкулеза, но отрицательными результатами микроскопии
мокроты на M. Tuberculosis;
4. Пациенты, плохо отвечающие на противотуберкулезную терапию.
