1) The study compared postoperative pulmonary function and pain control in COPD patients undergoing open abdominal aortic aneurysm repair with either epidural (Group I) or intravenous (Group II) analgesia.
2) Pulmonary function test (FEV1 and FVC) results were significantly better preserved in Group I patients on postoperative days 1 and 4.
3) Group I patients also had significantly less reported pain on postoperative days 1, 2, and 4 both at rest and during activity.
4) There were no differences in length of hospital stay, morbidity, or mortality between the groups.

The study included 40 cases, planned for operative laparoscopy for benign gynaecologic indications. Cases were randomly allocated to one of the 3 designed groups; Group-A (10 cases), where passive rapid gas deflation was done, group-B (15 cases), where passive slow gas deflation was done, and group-C (15 cases), where gas was rapidly and actively deflated. Postoperative pain was assessed by Visual Analogue Scale (VAS) at 1, 6, 24 hours and 7 days after operation. Postoperative analgesic requirements and frequency of nausea and vomiting were also recorded. The overall results showed that postoperative VAS was significantly less in group C. Total amount of analgesia needed in group C and B was significantly less compared to group A (p=0.0001). We concluded that active aspiration of residual gas at the end of gynaecologic laparoscopy significantly reduces postoperative pain. This simple clinical manoeuvre may also reduce postoperative nausea and vomiting.

The most common and distressing symptoms, which follow anaesthesia and surgery, are pain and emesis. The consequences of PONV are physical, surgical and anesthetic complications for patients as well as financial implications for the hospitals or institutions. Sometimes nausea and vomiting may be more distressing especially after minor and ambulatory surgery, delaying the hospital discharge. Laparoscopic surgery is one condition, where risk of PONV is particularly pronounced due to pneumo-peritoneum causing stimulation of mechanoreceptors in the gut. In spite of plenty of anti-emetic drugs available no single drug is 100% effective in prevention of PNV and combination therapy has got a lot of side effects.

Background: Pneumonia associated with mechanical ventilation (VAP) is one of the important
causes of nosocomial infections in pediatric intensive care units (PICU). VAP is the leading cause of morbidity
and mortality in PICUs. Aim: To assess the compliance to ventilator bundle components: elevation of the head
of bed >30, sedation interruption, spontaneous breathing trial, peptic ulcer prophylaxis and its effect on the
prevention of VAP. Subjects and Methods: A case control study at PICU of Abo EL Reish El Moneira Hospital,
including all mechanically ventilated patients admitted over a period of one year. The study tested the effect of
implementation of this bundle as regard the rate of VAP in both group, compliance to bundle and most affecting
component of it. Results: There was decrease incidence of VAP after implementation of the bundle, from (50%)
to (14%). Development of VAP was mostly affected by being in supine position, long duration of mechanical
ventilation and presence of pump failure. (p<0.05) The compliance to bundle components was statistically
significant, p= 0.001. Conclusion: VAP rate decreased after implementation of this bundle. Elevation of the
head of bed was the most compliant component of bundle in the PICU.

Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.

STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Background: We reviewed our experience to determine the effect of epidural versus intrave-
nous analgesia on postoperative pulmonary function and pain control in patients with chronic
obstructive pulmonary disease (COPD) undergoing open surgery for abdominal aortic
aneurysm.
Methods: A retrospective study with prospective collection of data of 30 COPD patients under-
going open abdominal aortic aneurysm repair, during a 5-year period. Group I (n ¼ 16) was oper-
ated under combined general and epidural anesthesia and epidural analgesia; group II (n ¼ 14),
under general anesthesia and intravenous analgesia. All patients performed pulmonary function
tests (PFTs) preoperatively and during postoperative days 1 and 4. Pain assessment was per-
formed on all patients during rest and activity on postoperative days 1, 2, and 4 by using the
visual analog scale. Data were recorded for PFTs, postoperative pain, length of hospital stay,
length of ICU stay, and postoperative pulmonary morbidity, including atelectasis and pulmonary
infections.
Results: There was no in-hospital mortality. Hospital stay was similar between the two
groups (group I: 7.1 ± 1.0, group II: 7.5 ± 1.1). Group I patients showed significantly
increased postoperative PFT values compared with group II patients at all time points (post-
operative day 1: FEV1(%): 32.3 ± 4.4 vs. 27.1 ± 1.6, p ¼ 0.007, FVC(%): 35.4 ± 8,5 vs.
28.3 ± 2.3, p ¼ 0.035; postoperative day 4: FEV1(%): 50.4 ± 6.8 vs. 41.9 ± 6.8, p ¼
0.017, FVC(%): 51.3 ± 8.3 vs. 43.0 ± 7.9, p ¼ 0.046). However, postoperative clinical
pulmonary morbidity was not different between groups. Group I patients showed signifi-
cantly reduced postoperative pain at all time points compared with group II patients. These
differences were more pronounced during postoperative days 1 and 2, both at rest (visual
analog score: 1.1 ± 0.9 vs. 2.6 ± 1.6, p ¼ 0.02 and 0.7 ± 0.8 vs. 1.9 ± 1.1, p ¼ 0.021,
respectively) and during activity (2.3 ± 0.8 vs. 4.0 ± 1.7, p ¼ 0.013 and 1.6 ± 0.7 vs.
