1215 austin warters integrated care how innovation can improve services editinvestnethealthcare
This document discusses integrated care and re-ablement approaches. It defines integrated care as coordinating health and social care services around an individual's needs and goals. Re-ablement is described as a short-term, intensive intervention delivered in the home to improve independence through occupational therapy, physiotherapy, and skilled home care workers. An evaluation of Dublin's re-ablement project found that after the intervention, 86% of service users improved in self-care, 66% in day-to-day activities, and 69% in mobility. Re-ablement also significantly reduced users' ongoing homecare needs. While costly to implement, re-ablement achieves better outcomes for service users and may reduce long-term healthcare
This document discusses strategies for improving the sustainability of the Dutch healthcare system. It suggests that system innovation, rather than just process innovation, is needed. Concentrating specialized care into fewer locations while decentralizing other services could improve quality and lower costs. Case studies show relationships between higher volume and better outcomes. The document also discusses lessons from disruptive innovation theories and examines models for reorganizing the healthcare system into specialized centers, primary/chronic care, and acute care networks to make the system more affordable and accessible over the long run.
Dr Jennifer Dixon: Commissioning and integrated careNuffield Trust
Commissioning and integrated care aims to improve coordination and alignment of incentives across health and social care. 3) The pilot demonstrated that a regional ECR is achievable and could help improve patient safety, quality of care and reduce costs across Northern Ireland.
NHS Scotland Ehealth Strategy - Alan Hyslophealthcareisi
This document summarizes Scotland's approach to eHealth and strategy for a national electronic health record system. Some key points:
- Scotland has a federated health system with 14 regional health boards and over 1000 GP practices.
- The strategy focuses on incremental improvements, partnerships, and ensuring the right information is available to clinicians.
- Key early successes included a unique patient ID, online test results, and a national emergency care summary covering 99.9% of the population.
- Future areas of focus include electronic records for palliative care patients and a "key information summary" to facilitate coordinated care across providers.
Multidisciplinary Team Experience In The Incorporation Of Pacs And Realtime E...healthcareisi
The multidisciplinary team (MDT) at St. James's Hospital experienced significant increases in workload due to the implementation of PACS, electronic patient records, and an increase in patient referrals. To accommodate the workload, the MDT increased meeting time from 1 to 1.5 hours and was able to discuss 24 patients per hour compared to 14 previously. They coped through strategies like grouping similar cases, prioritizing, and incorporating electronic record keeping into their meetings. However, this reduced time spent reviewing images and clinical summaries for each patient.
This document summarizes the findings of a data protection and quality review at a medical clinic. It identifies some positive aspects but also notes 12 areas of concern and 6 critical risks to compliance. Recommendations include formalizing governance structures and policies, role-based access controls, training programs, and transitioning project management practices to ongoing governance. Implementing these changes would help ensure all staff properly handle private patient data in accordance with regulations.
Making Sense Of Ubiquitous Health Information A Cross Generational Study - Ka...healthcareisi
This study examined how individuals across generations perceive emerging health technologies that collect and share personal health information. Younger adults saw value in sharing to motivate each other and craft their online identities. Middle-aged adults were open to sharing within existing social circles. Older adults preferred sharing only with family and for research purposes rather than broadcasting. Designers need to consider these generational differences in privacy concerns, social relationships, and how technology metaphors are understood differently by age.

"Does Foreign Direct Investment Negatively Affect Preservation of Culture in the Global South? Case Studies in Thailand and Cambodia."
Do elements of globalization, such as Foreign Direct Investment (FDI), negatively affect the ability of countries in the Global South to preserve their culture? This research aims to answer this question by employing a cross-sectional comparative case study analysis utilizing methods of difference. Thailand and Cambodia are compared as they are in the same region and have a similar culture. The metric of difference between Thailand and Cambodia is their ability to preserve their culture. This ability is operationalized by their respective attitudes towards FDI; Thailand imposes stringent regulations and limitations on FDI while Cambodia does not hesitate to accept most FDI and imposes fewer limitations. The evidence from this study suggests that FDI from globally influential countries with high gross domestic products (GDPs) (e.g. China, U.S.) challenges the ability of countries with lower GDPs (e.g. Cambodia) to protect their culture. Furthermore, the ability, or lack thereof, of the receiving countries to protect their culture is amplified by the existence and implementation of restrictive FDI policies imposed by their governments.
My study abroad in Bali, Indonesia, inspired this research topic as I noticed how globalization is changing the culture of its people. I learned their language and way of life which helped me understand the beauty and importance of cultural preservation. I believe we could all benefit from learning new perspectives as they could help us ideate solutions to contemporary issues and empathize with others.

2. Defining integrated care
“[Integrated care] is a concept bringing together
inputs, delivery, management and organisation of services
related to diagnosis, treatment, care, rehabilitation and health
promotion.”
(Grone & Garcia-Barbero: 2001)
“[Integrated care] is a coherent set of methods and models on
the funding, administrative, organisational, service delivery and
clinical levels designed to create connectivity, alignment and
collaboration within and between the cure and care sectors.”
(Kodner & Spreeuwenberg: 2002)
HISI Annual Conference, Dublin, November 16th 2011
3. Why we need integrated care?
Because of our failings in current health and social care systems:
– Addressing the changing demand for care
– Poor efficiency fragmented approaches (disease versus
patient)
– Improving the quality and continuity of care
– Recognising that health and social care outcomes are
interdependent
– Exclusion of vulnerable groups
(Wait, European Social Network Conference, Edinburgh 2005)

18. A new standard?
Yes!
“...An integrated care pathway with
flexible shared-care arrangements
between primary care and
hospital, facilitated by information
technologies, has an enormous potential
to decrease hospital admissions in chronic
obstructive pulmonary disease patients.”
HISI Annual Conference, Dublin, November 16th 2011
19. A new standard?
But!
“... In the integrated care approach,
several models are available and we now
require comparative studies.”
“We must also be aware that from a
healthcare provider perspective, further
studies of this model must include cost-
effectiveness as well as effectiveness in
older disabled patients with chronic
obstructive pulmonary disease”
HISI Annual Conference, Dublin, November 16th 2011
20. What had we learnt (2006)?
• There were better alternatives to the current approach of
chronic patient management.
• These alternatives were safe, cheaper and generated
better clinical outcomes and professional and patient
satisfaction.
• Information technologies facilitated some of the aspects
related to their implementation.
• Most of the difficulties concerned the organisation of
care delivery and professionals.
• There was a need to change our viewpoint
