Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.

CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration

2. Research and Planning
Safari
During the research and planning phase, we used the Safari web browser in order to use the
internet to aid our research, this is because Safari is the default browser on Apple Mac’s. I
found Safari difficult to use because of the more ‘clustered’ appearance when switching
between tabs. I often got confused with what tab was what and therefore I began getting
frustrated using the browser which sometimes hindered the quality of my research. Despite
this, I did enjoy the easy ability to add bookmarks to the toolbar which meant I could quickly
access my favourite and most useful resources.
3. Research and Planning
Blogger
Blogger was an essential tool that we used to document and track all the
research and planning we did. I had no problems using Blogger due to my
experience using it for my foundation portfolio. I enjoy the ability to
embed files such as audio and video tracks from external sources as this
allows me to add a variety of different media to my blog and make it a lot
more entertaining to read through.
4. Construction
Adobe Premier
We needed to use a form of editing software in order to import clips and edit them to put
together our documentary. We edited our clips on the “Timeline” which is a feature of Adobe
Premier. The timeline provided us with a way to put together our clips and reduce them to a
size and speed which we found suitable for our documentary. This feature was where we
would spend most of our time when using Premier and thought it was an essential part of
our construction process.
One way which we used the timeline was to visually adjust clips using the pen tool. This
allowed us to adjust the brightness of the clips when needed. By doing this we were able to
achieve a consistent brightness level throughout our documentary which therefore adds to
the level of professionalism our documentary represents.
5. Construction
Adobe Premier
Another feature was the use of titles. The titles allowed us to
follow codes and conventions of documentary’s by using
opening headings and also supporting headings during the
expert interviews to give our audience context to the
interview.
6. Construction Conclusion
Prior to this advanced portfolio, I have had no experience
using Adobe Premier or any other video editing software, so
everything I now know what to do has been learnt during the
whole construction process. Overall I found Adobe Premier a
little confusing at first, due to the severe lack of user guidance
built into the software and so therefore this led to a lot of trial
and error by myself in order to get the result I wanted.
However, once I became more familiar with the software and
I knew the different keyboard shortcuts available to me, the
whole editing process became a lot simpler and I was able to
do things a lot quicker than I did in the beginning and so this
meant I really enjoyed using this software and learning to use
a good chunk of the features available to me.
