This document contains an evaluation form for observing a teaching demonstration. The form has several criteria for evaluating the candidate's preparation, oral communication, physical presence, class management, use of visual aids, handling of student questions, presentation of the lesson's purpose, knowledge of the subject matter, ability to engage students, conclusion of the lesson, confidence, time management, and inclusion of all students. The evaluator rates the candidate in each area as strong, good, fair, or weak and provides any remarks on their performance. The evaluator then signs and prints their name at the bottom.
