It collects information on the class, content, number of students, average student age, and rates the professor on their presentation, communication skills, classroom management, materials, organization, creativity, questioning, explanations, learning environment, and feedback. It also rates the students on participation, teamwork, pronunciation, respect, grammar, understanding, responsibility, time management, and self-evaluation. Space is provided for additional notes.
