The "diamond concept" refers to providing the essential elements for bone healing - osteoinductive mediators, osteogenic cells, an osteoconductive scaffold, optimal mechanical stability, vascularity, and addressing host factors. A review of studies found applying all aspects of the diamond concept to treat long bone non-unions achieved 89-100% union rates, while applying fewer principles resulted in 44-90% success rates. The concept offers a biological approach beyond mechanical stability alone by augmenting the fracture site.
