This document discusses a case of occult spinal dysraphism with diastematomyelia and a low tethered spinal cord in an 11-year-old male patient. Radiological findings from X-ray, CT myelography, and MRI show splitting of the spinal cord separated by a bony spur, as well as a dilated subarachnoid space. Occult spinal dysraphism is described as a heterogeneous group of disorders involving anomalies of the spinal cord and meninges covered by an intact skin. Causes may include defects in primary or secondary neurulation leading to conditions like lipomas, tight filum syndrome, and dorsal dermal sinuses. Clinical presentation can include orthopedic deformities and neurological symptoms
