A 27-year-old female patient visited the dentist with a toothache in her lower left back teeth for one month. Upon examination, the dentist found a periodontal pocket between teeth 36 and 37, with tooth 37 being loose and tender. Tooth 37 also showed signs of fusion with an extra tooth. Due to the improper size and positioning of the fused teeth, there was a gap between the first and second molars, causing localized periodontitis and a periapical abscess. The patient opted to have tooth 37 extracted. Examination of the extracted tooth revealed it was a fused second molar with a supernumerary tooth, which is a rare occurrence of fusion in the permanent posterior teeth.
