Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration

Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)

YOUR TEAM RESPONSE
MY name is CDS and today we are going to talk about streamlining patient care into your built environment. I am talking about when you are building out a MRI, CAT Scan or EP Cath Lab suites what is your role and why it is important you put time and effort into this new project that has been dropped on your lap. Yes I realize Yet one more responsibility but we are going to show you how to use a simple patient centric strategy that gives you the ability to take control of your capital imaging projects get them done on time and on budget without forgetting about the patient and your staff.
Each of you have enough responsibility on your plates to begin with and when a project is added to your responsibilities you want to be able to be prepared to manage it and not stress out. Projects are easy until you add in the people that is when you have the drama. etc. When you work on a team what will your role be and what roles will the other people play? How will the team be inspired and is there a return on everyone’s involvement?
So if you listen very carefully today you will walk away with a method to develop your own skills at the same time giving you the creative license to frame your culture and personal style. What I mean by this is each of you have your own work style and the healthcare organization has a unique culture, geography and patients. If you take the time to use the process and tools when you are tasked with buying and installing a new MRI, Cat Scan or EP Cath lab to name a few all you need to do is think of three steps:
Let me begin with my own patient story. For many years I lead teams of professional medical and in the construction field to specifically build-out patient centered environments. I’m not exactly one of your normal people. I was born into a chaotic family (really the stork dropped me at the wrong house) with the project management gene so as much as I believed in making sure our teams took the time to think through the patient experience but deep down inside I wanted control over the budget and the schedule. Sometimes that may have taken the lead.
All that changed when I became a patient because I went from being a fast paced health care executive in total control to sitting in a hospital bed with no control. I went from leading teams to needing teams to do every little thing. I went from fast paced to no pace and when things slowed down I began to notice more things. I noticed the nurses and doctors every move, what they tripped over, whether or not they smiled and showed warmth. I noticed noises every single noise, I notices smells, cleaners, were they green or strong pungent, food- guess what we are having for dinner tonight, flowers yes they were beautiful. Think about a Sunday morning sitting in an outdoor café watching people scurry along the street, you see them try to figure out their story while listening to the conversations, street sounds and feeling the breeze.
One of the most important thing I noticed was that I worked in healthcare for 14 years and I thought I knew healthcare but I had much more to learn. At this very moment is when I decided I needed to spread the word that all of us might very well be in the same situation and when we slow down and have time to look around what will we see, feel, smell? The thing that stays on our mind is are we safe, can we trust our caregivers and will we be cared for in the very best possible manner in this vululable state. It starts with the environment. When you go to someones house and look around if things are clean, organized and present well you have a sense of confidence that these people care. They care about themselves. Each and every one of us may be sitting in a hospital bed looking around thinking who the heck built this place. My story may be your story one day so it really matters that we spend our capital dollars wisely. Let’s get this done.
I remember it was a beautiful day in May when I was sitting at the hospital BBQ picnic table eating corn on the cob and BBQ Chicken with Phil and Jackie my work mates. Little did I know that by the end of the day I would be scheduled for a Cat Scan and surgery. One minute I was enjoying the sun and food and the next minute the doctor is telling me she is going to use the DeVinc robot to do surgery to suck the tumor out that they discovered in my annual checkup. Dr. Onyx did state that things may change once she got the result of the Cat Scan. Change they did because once she saw that the tumor was pushing up on my bladder she said sorry I have got to get in there and get my hands around it.
Here I was.. never really had any medical issue let alone a Cat Scan. When I needed to schedule a Cat Scan I got to choose where I could schedule my Cat Scan and I decide to have that done at the Darien Imaging Center because I lead the team to build that center.
I knew it well and was impressed with the staff when I handed the keys over to the manager Beth and her she was committed to taking care of the place because they were involved in the project from start to finish. Beth met with her staff while they shared thier wisdom and experience and felt empowered because they could see her own ideas and suggestions built right into the environment. Being on the team we all built relationships as we moved toward our vision a soothing, warm and relaxing environment that would serve this affluent community. An imaging center like no other. With an outdoor terrace, full glass windows, beautiful wood reception desk with a fish tank to screen patients admissions. Artwork that taught about the glaciers and brought the outdoors in along with a café style area with a TV with snacks and drinks. Yes from the minute I walked into the place I felt in trusted hands. I trusted that they would take good care of me. And they did. How did we know in the beginning of designing and planning this place that it would turn out so well? We didn’t but we focused on the patient, picked the right team, make sure everyone on the team was engaged by taking the time to listen to what they were saying. We created an environment rich with stories so we each got to know each other and what was important to us as a person and our profession. You see stories equal connections, equal relationships which equals better people and workplaces. All of this inspires action. This is why I create a storytelling tool to help each of you use your stories to create relationships and connections. Called the SPA tool – Story what are the points of the story and how do we apply the story to the design, infection prevention and or logistics.
