At-Home PT, OT & ST Services
• Outside nursing services & home bound status are not required.
Customized, Need-Based Programs
• Upon evaluation, determinations are made based on baseline
function and cognitive evaluation
Quarterly Screens
• Includes identifying cognitive and function declines. This provides
an opportunity for further evaluation for physical and occupational
and speech therapy intervention. This includes cognitive evaluation,
staging, and customized strategies and education provided by our
Speech/Language Pathologists to support a patient’s best ability to
function.

• Every therapist that enters your campus has been trained on the latest in-home
regulations and has a knowledge base that marries techniques used in home health,
skilled nursing, and outpatient therapy.
• All therapists must complete our rigorous and competency-based training program

9. Intake and Billing
Billed under Medicare Part B
• Intake form for all patients is required before treatment
• We secure the physician’s order for therapy services
• We work with a third-party billing company to verify insurance benefit and
obtain any required authorizations
• If a co-pay is required, this is discussed with the resident/POA prior to start
of treatment plan
• We bill all therapy services and handle any claim denials through our denial
management department
