- 32. การช่วยหายใจด้วย face mask 1.ทรวงอกขยายขยับขึ้น-ลงตามการบีบ bag ตัว bag ไม่แฟบ สีผิวผู้บาดเจ็บหรือผู้ป่วยแดงดี และ O2 saturation ดีขึ้น 2. ไม่มีเสียงลมรั่วออกจากหน้ากาก 3. ไม่มีแรงต้านมากเกินไปขณะบีบ bag ผศ.ดร.กรองได อุณหสูต 1 ตุลาคม 2565 B

Face mask แล้วยังพร่องออกซิเจน 1. เปิดทางเดินหายใจใหม่ ด้วยวิธี jaw thrust 2. ใส่ oropharyngeal หรือ nasopharyngeal airway 3. ใช้สองมือจับหน้ากากครอบให้สนิท แล้วให้ผู้ช่วยบีบ bag แทน 4. เปลี่ยนขนาดของ mask ให้เหมาะกับใบหน้าของ ผู้บาดเจ็บหรือผู้ป่วยฉุกเฉินมากขึ้น 5. ประเมินสิ่งแปลกปลอมในทางเดินหายใจซ้า และ clear airway 6. ถ้ายังไม่ดีขึ้น รีบรายงานแพทย์ และเตรียมช่วยแพทย์ใส่ ท่อช่วยหายใจ ผศ.ดร.กรองได อุณหสูต 1 ตุลาคม 2565
- 34. เตรียมอุปกรณ์ใส่ท่อช่วยหายใจ 1. ออกซิเจนและอุปกรณ์ช่วยการหายใจ ได้แก่ ออกซิเจนSelf-inflating bag หรือ AMBU bag, airway และ face mask ที่มีขนาด เหมาะสม 2. Laryngoscope มีไฟที่ปลาย blade พร้อมใช้ อาจเป็นชนิดโค้ง (Macintosh blade) หรือ ตรง (Miller blade) ผู้ใหญ่นิยมใช้ blade โค้ง เบอร์ 3 หรือ 4 ผศ.ดร.กรองได อุณหสูต 1 ตุลาคม 2565
- 35. เตรียมอุปกรณ์ใส่ท่อช่วยหายใจ 3. ท่อช่วยหายใจ cuffไม่มีการรั่วซึม ขนาดพอดี ใหญ่กว่า และเล็กกว่า อย่างละ 1 ท่อ กระบอกฉีด ยา ขนาด 10 มิลลิลิตร stylet เพื่อดัดท่อช่วย หายใจ gel ไว้หล่อลื่น เทปเหนียว หรือเทปผูก ท่อช่วยหายใจ 4. เครื่องดูดเสมหะ (Suction) และสายดูดเสมหะ 5. ยาหย่อนกล้ามเนื้อ ยาระงับปวด หรือยาดมสลบ ตามแผนการรักษา ผศ.ดร.กรองได อุณหสูต 1 ตุลาคม 2565
