Manyasthambha is one among the Shoola and Sthambha Pradhana Vataja Nanatmaja Vyadhi, intervening with the functional ability of neck and upper limb. Cervical spondylosis is the major cause of morbidity throughout the world affecting mainly aged and young adults also. Life time incidence of Manyasthambha is more than 40 . In the present day, man expects miraculous effects with treatment which are easily available with more efficacy and less restrictions in routine work. In this present Case study, a diagnosed case of Manyasthambha has been included for its ayurvedic management. Chief complaints were pain and stiffness over the neck for 3yr and pain over the neck was radiating towards right shoulder. Abhadya choorna was given with ushna Jala as Anupana for 30 days and assessment done before and after treatment with help of standard gradings, there is a complete relief in parameters like pain and moderate relief in stiffness and with added benefits like increase in appetite, relief from constipation and good sleep was observed. Back Pain
Back pain is often a common symptom of many disease conditions and the back pain may range from simple or dull pain to sudden and sharp pain. If the pain persists for few days, it is acute pain whereas if continues for more than 3 months, it is considered as chronic pain. In most cases, back pain may resolve without any treatment however if persists for more than 3 days, medical intervention is necessary.
Neck Pain
The first 7 vertebral bones on the spinal column form the cervical spine and are located in the neck region. The neck bears the weight of the head, allows significant amount of movement, and also less protected than other parts of spine. All these factors make the neck more susceptible to injury or other painful disorders. Common neck pain may occur from muscle strain or tension in everyday activities including poor posture, prolonged use of a computer and sleeping in an uncomfortable position.
Spinal Deformity Surgery
The Spine or backbone provides stability to the upper part of our body. It helps to hold the body upright. It consists of several irregularly shaped bones, called vertebrae appearing in a straight line. The spine has two gentle curves, when looked from the side and appears to be straight when viewed from the front. When these curves are exaggerated, pronounced problems can occur such as back pain, breathing difficulties and fatigue and the condition will be considered as deformity. Spine deformity can be defined as abnormality in the shape, curvature and flexibility of spine.
Spine Injections
Spine injection is a nonsurgical treatment modality recommended for treatment of chronic back pain. Injection of certain medicinal agents relieves the pain by blocking the nerve signals between specific areas of the body and the brain. The treatment approach involves injections of local anaesthetics, steroids, or narcotics into the affected soft tissues, joints, or nerve roots. It may also involve complex nerve blocks and spinal cord stimulation.
Spine Trauma
Spine trauma is damage to the spine caused from a sudden traumatic injury caused by an accidental fall or any other physical injury. Spinal injuries may occur while playing, performing normal activities, operating heavy machines, lifting heavy objects, driving automobiles, or when you suffer a fall. Injury to spine may cause various conditions including fractures, dislocation, partial misalignment (subluxation), disc compression (herniated disc), hematoma (accumulation of blood) and partial or complete tears of ligaments.
Vertebral Fractures
Vertebral compression fractures occur when the normal vertebral body of the spine is squeezed or compressed. The bone collapses when too much pressure is placed on the vertebrae, resulting in pain, limited mobility, loss of height, and spinal deformities. In severe compression fractures the vertebral body is pushed into the spinal canal which will apply pressure on the spinal cord and nerves.

Living a sedentary lifestyle – Individuals who rarely
if ever engage in physical activity are more prone to
herniated discs because the muscles that support
the back and neck weaken, which increases strain
on the spine.
Traumatic injury to lumbar discs-
commonly occurs when lifting while bent at the
waist, rather than lifting with the legs while the
back is straight.

Practicing poor posture – Improper spinal

weakening of the disc.
Mutation- in genes coding for proteins involved
in the regulation of the extracellular matrix, such
as MMP2 and THBS2, has been demonstrated
to contribute to lumbar disc herniation.

When the spine is straight, such as in standing or lying

internal pressure is equalized on all parts of the discs.
While sitting or bending to lift, internal pressure on a disc
can move from 17 (lying down) to over 300 psi

9. Disc herniation can occur in any disc in the spine,
but the two most common forms are lumbar disc
herniation and cervical disc herniation.
The former is the most common, causing lower
back pain (lumbago) and often leg pain as well, in
which case it is commonly referred to as sciatica.
Lumbar disc herniation occurs 15 times more often
than cervical (neck) disc herniation, and it is one of
the most common causes of lower back pain.
The following locations have no discs and are
therefore exempt from the risk of disc herniation: the
upper two cervical intervertebral spaces, the
sacrum, and the coccyx.
Epidemiology
10. Most disc herniations occur when a person is in their
thirties or forties when the nucleus pulposus is still a
gelatin-like substance.
With age the nucleus pulposus changes ("dries out") and
the risk of herniation is greatly reduced.
After age 50 or 60, osteoarthritic degeneration
(spondylosis) or spinal stenosis are more likely causes
of low back pain or leg pain.
4.8% males and 2.5% females older than 35
experience sciatica during their lifetime.
Of all individuals, 60% to 80% experience back pain during
their lifetime.
In 14%, pain lasts more than 2 weeks.
Epidemiology
11. The patient has severe back pain and is
unable to straighten up.
Radiating pain to the buttock and lower
limbs and is associated with paraesthesia
or numbness in the legs or foot( sciatica)
and occasionally there is muscle
weakness.
Both backache and sciatica are made
worse by coughing or straining.
Cauda equina compression is rare but
may cause urinary retention and perineal
numbness.
Clinical Features
12. The patient usually stands with a slight list to
one side(‘sciatic scoliosis’).
Sometimes the knee on the painful side is held
slightly flexed to relax tension on the sciatic
nerve; straightening the knee makes the skew
back more obvious.
All back movements are restricted, and during
forward flexion the list may increase.
Clinical Features
13. There is often tenderness in the midline of
the low back, and paravertebral muscle
spasm.
Straight leg raising is restricted and painful
on the affected side; dorsiflexion of the foot
may accentuate the pain.
Sometimes raising the unaffected leg
causes acute sciatic tension on the painful
side (‘crossed sciatic tension’).
With a high or mid-lumbar prolapse the
femoral stretch test may be positive.

Do not bend, lift, or sit in a soft, low chair; the pain
will get worse.
The affected individual may feel better lying on his
or her back on a firm surface with a pillow under
his or her knees. Another option is lying on one's
side with a pillow between the knees to keep the
back straight.
Take it easy, but do not simply lie in bed because
this has been shown to actually worsen the
condition. Do activities one is able to tolerate, and
do not expect to feel better overnight.
