There was an increasing trend in the number of total knee arthroplasty (TKA) operations performed and patients receiving TKAs from 2000 to 2009. While the functional status and severity of patient symptoms did not change significantly over this period, there was a decrease in the mechanical axis deviation, indicating less severe radiographic osteoarthritis. The number of elderly patients over 80 years of age receiving TKAs increased significantly, but the number of younger patients under 60 did not change.

Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty.
We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system.
In group of Vancouver A fractures, 3 patients were treated with a mean score of 15,7 points (good result). We recorded a mean score of 14,2 points (fair result) in 6 patients with Vancouver B1 fractures, 12,4 points (fair result) in 24 patients with Vancouver B2 fractures and 12,8 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16,2 points (good result) in 7 patients.
Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable and checking the stability of the prosthesis fixation during surgery should be performed.

total knee arthroplasty on this tibia with a “malunion” presents technical difficulties related to the initial approach, the presence of osteosynthesis material, the presence of malunion and the change of bone density. The objectives of this study are to determine the clinical and radiographic results of patients undergoing Total Knee Arthroplasty (TKA) after High Tibial Osteotomy (HTO). This is a retrospective descriptive study including patients undergoing Total Knee Arthroplasty (TKA) after an High Tibial Osteotomy (HTO) at the Hospital of Mont de Marsan (France) from 2008 to 2017 with a minimum follow-up of 12 months. Thirty knees (27 patients) were recruited. The sex ratio was 1.72. The average age was 70.33 years (54years-88years). The average time between High Tibial Osteotomy (HTO) and Total Knee Arthroplasty (TKA) was 10.83 years (1 year-26 years). The medial opening was 63.33% and lateral closure for the rest. Clinical improvement was observed, with an average gain of 24.97 points for pain, 1 point for stability, 1 point for knee mobility and 5 points for walking distance. The clinical result was perfect in 13.33%, excellent in 42% and medium in 36.67% of cases. The alignment was obtained in 76.67% of cases (p = 0.0039). The posterior tibial slope, epiphyseal varus, patellar height were corrected in 80% of cases respectivly (p = 0.000011, p = 0.44, p = 0.15). Residual pain was observed in 26.66%, joint stiff ness in 16.66%, skin healing disorder in
16% and infection in 6.66% of cases. Total knee arthroplasty made it possible to recover the failure of an high tibial osteotomy.
A Study to Evaluate Spatial Gait Parameters in Patients of Osteoarthritic KneeIOSR Journals
Abstract: Osteoarthritis is a slowly evolving articular disease, which appears to originate in the cartilage and
affects the underlying bone, soft tissues and synovial fluid. Quantitative gait analysis is an important clinical
tool for quantifying normal and pathological patterns of locomotion, and has also been shown to be useful for
prescription of treatment as well as in the evaluation of the results of such treatment. The objectives of this study
was to evaluate spatial gait parameters variations in patients with osteoarthritic knee as compared to normal
healthy individuals. 100 subjects were included in the study, 50 normal healthy subjects and 50 subjects with
bilateral knee osteoarthritis with age group of 40-60 years based on the inclusion and exclusion criteria. Before
gait analysis, all subjects were given informed consent as advised by the Institutional Ethical Committee. Gait
analysis was done for both groups with same protocol. Following gait parameters was measured individually
for each subject: Step length [m], Stride length [m], Normalised stride length [%], Step width [m] and Foot
angle [degree]. Paired and unpaired t- test will be used for the analysis of data. The study concluded that there
was significant difference is spatial gait parameters in patients with osteoarthritic knee as compared to normal
healthy individuals.

the last few decades. Starting from an era of no surgery,
it has refined to minimally invasive surgeries and keyhole
surgeries in almost all sub specialties of Orthopaedics.
Total hip arthroplasty has been an important surgical operation in orthopaedics in the 20th century. After many trails, major advancement in Total Hip Arthroplasty was made by Sir John Charnley in 1962, who introduced low friction arthroplasty. This consists of a polyethylene cup and 22.2 mm head, both components being fixed with methacrylate cement. In the following years there were many changes to this basic principle (model) of total hip arthroplasty. Patient education has become an important factor in improvement of function following total hip replacement.
We report the 11-year follow-up of a premenopausal woman with osteogenesisimperfecta (OI) who
was treated with alendronate. A 41-year-old Japanese premenopausal woman with OI type I who had
frequently experienced painful fragility fractures consulted our clinic because of chronic back pain associated
with spinal osteoporosis. She had undergone heart surgery (aortic valve replacement) because of aortic
regurgitation 5 years before her first consultation with our clinic. After surgery, she began taking warfarin (3
mg/day), and this treatment was continued during our follow-up period. She was treated with alendronate (5
mg/day or 35 mg/week) for 11 years. The patient’s urinary cross-linked N-terminal telopeptides of type I
collagen and serum alkaline phosphatase levels decreased, while the bone mineral density of her lumbar
spine (L2–L4) increased, as measured using dual energy X-ray absorptiometry. The serum calcium and
phosphorus levels stayed within the normal ranges. Three non-vertebral fractures occurred at the hip, ankle,
and ring finger during the 11-year treatment period, but no adverse effects were observed. Thus, the present
case report showed the long-term outcome and safety of alendronate treatment in a premenopausal woman
with OI type I.

Setting A tertiary referral centre for joint replacement surgery in a
teaching hospital in Hong Kong.
Patients All patients who underwent primary total knee arthroplasty for
primary knee osteoarthritis from January 2000 to December
2009.
Results In all, 1157 total knee arthroplasties (589 left and 568 right)
were performed on 588 females and 162 males. The annual
number of total knee arthroplasties increased from 91 in 2000
to 181 in 2009. The annual number of patients increased from
58 (46 female, 12 male) in 2000 to 159 (117 female, 42 male) in
2009. When compared yearly results, there were no significant
changes in the preoperative Knee Society Knee Score, Knee
Society Functional Assessment, and passive range of motion of
these patients. However, there was a significant decreasing trend
with regard to lower limb mechanical axis mal-alignment, from
15.1º deviation from the neutral axis in 2000, to 14.8º deviation in
2004, and then 12.9º deviation in 2009 (mostly varus deformity).
There was no difference between left knees and right knees,
and between females and males. The mean age of the patients
did not show significant change over the past decade, but the
number and proportion of patients over 80 years old showed an
increase from 4.8% (2000 to 2004) to 13.8% (2005 to 2009). On
the other hand, the number and proportion of patients under
the age of 60 years did not change.
Conclusions There was an increasing trend towards total knee arthroplasties,
both in terms of number of operations and patients. The number
of younger patients having total knee arthroplasty did not
increase over the last 10 years, whereas the number of those
older than 80 years increased significantly over that period.

Hoaglund et al10 studied the radiographic
prevalence of knee OA in 500 hospitalised patients 20
in QMH in 1967. Using the Kellgren Lawrence grade Hospital Authority

of 5% in local men and 13% in local women. Recent
reports of population-based studies in Beijing and 15
Shanghai revealed that the prevalence of knee OA in
Chinese people was similar, if not higher than that in
their Caucasian counterparts.2,11 There was no good
explanation for the drastic increase, but current % 10
evidence proved that the Chinese population did not
differ from the western populations in terms of the
prevalence and severity of knee OA.
The traditional Chinese belief in bodily 5
conservation may deter some elderly patients from
having TKAs, but the public’s acceptance of the
operation is certainly increasing. The growing waiting
