Duke Endowment Grant   Navigating Frail Seniors from Hospital to Home Project Date:  01/01/09-12/31/11   Program Goals:  to develop a navigation and transitional care program for frail senior patients to increase awareness of and access to services and care among frail senior patients Measurable Results Reduce readmissions for the same condition within 30 days, by 10% Decrease unnecessary Emergency Department utilization by 10%
National Institute of Child Health and Human Development  Dissemination of a Tailored Injury Prevention Program to Facilitate Anticipatory Guidance in Pediatric Health Care Project Date:11/01/07-10/31/09  Program Goals: Determine barriers to the use and integration of the program in routine practice for well-child visits. Determine extent of program utilization during the first 3 months following implementation and at 6 month follow up period. Assess the effect of program implementation on provider-parent injury prevention anticipatory guidance as documented in the medical record. Assess differences in program implementation as a function of delivery system – handheld tablet versus computer unit. Assess differences in program implementation between clinic settings
Emergency Medicine Foundation   Emergency Department Screening and Intervention for Alcohol Related Problems   Project Date:  12/01/04 – 11/30/05   Project Goals: Injured patients screened positive by physicians in the emergency department using the American College of Emergency Physicians in their Alcohol (ACEP) screening tool will seek substance abuse treatment or primary care services within a year after the screening
