This is an audit I had done as Coordinator of acute pain service at Al Razi Hospital Kuwait. Through this I was able to draw attention to the rising rate of dislodgement and the technique of fixation was changed.
In 2013, Victoria Hospital's Day Surgery Unit (DSU) performed 354 cases, saving an estimated 552 bed days and generating $168,090 in revenue. The DSU, opened in October 2012, saw increasing utilization over the year from 40% bed occupancy in the first trimester to 72% in the final trimester of 2013. The presentation evaluated the DSU's history, case volumes, financial assessment, and concluded the unit was providing benefits in terms of reduced hospital stays and increased revenue for the hospital.

Laparoscopically assisted high ligation of patent processus vaginalis in children was evaluated in this study. 40 patients underwent the procedure, which involved inserting trocars and using laparoscopy to identify and ligate the patent processus with sutures tied externally. The procedure had a low complication rate, short hospital stay, and detected unsuspected contralateral hernias in 28% of cases. At 18-month follow up, there was one recurrence with no other complications, demonstrating this technique is a safe and effective minimally invasive option for treating hernias in children.

This study investigated the surgical benefits of using protoporphyrin IX (PpIX) fluorescence to detect meningioma tissue in 204 patients. The study found that PpIX fluorescence enabled visualization of most meningiomas and identification of residual tumor tissue. Titanium cranioplasty may be superior to autologous bone cranioplasty based on a randomized controlled trial comparing the two procedures. The trial found that titanium cranioplasty resulted in fewer implant failures requiring reoperation compared to autologous bone cranioplasty, though the difference was not statistically significant. Titanium cranioplasty also led to better reported functional and cosmetic outcomes with no difference in healthcare costs between the two groups over one year of follow up. However, the trial was prone to bias because the surgeon was not blinded to the treatment group assignment. Larger, multi-center studies are needed to verify the results.

This multicenter study examined 503 patients with recurrent glioblastomas who underwent surgical resection. It found that aggressive surgical resection of recurrent glioblastomas, when followed by adjuvant therapy, was associated with increased overall survival. Specifically, survival was longer for patients who were younger, had a higher Karnofsky performance status, underwent a more extensive resection, and had a longer time between initial diagnosis and recurrence. However, the study had limitations as a retrospective analysis subject to selection bias. Repeat resection improved survival, but its benefits compared to nonsurgical therapy require further prospective evaluation.

Katalu Mayu as an Hemostatic Agent after Tooth Extraction In Patients with Im...QUESTJOURNAL
Introduction: Many topically applied anticoagulants have been used to stop bleeding after tooth extraction. Ayurvedic product such as Katalu Mayu has shawn its efficacy for stoppage of bleeding after dental extraction. Materials and Methods: In this study, topical application of styplon powder was done over post extraction site in 300 patients, who are on oral anticoagulant therapy with prolonged bleeding time. Patients with age group of 30-50 years with 10-20 minutes of bleeding time suffering from systemic underlying illness like hypertension and taking acetyl salicylic acid and Warfarin were selected. Patients with systemic disease like hematologic, renal, or liver disease, bone marrow disorders or any concurrent medication affecting hemostasis were excluded. Dental extractions were carried out using local anaesthetic agent 2% Lignocaine with Adrenalin. No pressure pack was given and Katalu Mayu was applied in powder form on teeth sockets. Results: Out of 300 patients, 291 patients showed stoppage of bleeding within 60 seconds. Discussion: Dental extractions can be carried out without discontinuation of anticoagulant therapy. In such cases, the Ayurvedic product Katalu Mayu can be effective for stoppage of bleeding immediate after dental extractions. Conclusion: Katalu Mayu can be of effective remedies to stop bleeding immediate after dental extraction in patients who are suffering from hypertension and taking antihemostatic drugs.

Hysteroscopy showed high sensitivity (98.9%), specificity (97.5%), and diagnostic accuracy (98.3%) for detecting benign endometrial lesions but lower values for endometrial characterization, highlighting the need for biopsy for diagnosing conditions like hyperplasia and cancer. The study prospectively analyzed 105 patients undergoing laparoscopic cholecystectomy between May 2017 and January 2019. The study found that difficulty accessing the peritoneal cavity was associated with obesity and previous abdominal surgery. Bleeding during surgery was associated with previous upper abdominal surgery, thickened gallbladder wall, and history of acute cholecystitis. This study aimed to assess whether attaching the omentum (fatty tissue) to the stomach during laparoscopic sleeve gastrectomy (LSG) surgery could help reduce post-operative gastrointestinal (GI) symptoms like nausea and vomiting. The study involved 60 patients undergoing LSG who were randomly assigned to either have LSG alone or LSG with omentopexy. Patients completed surveys assessing GI symptoms at several time points after surgery. The study found that attaching the omentum did not significantly reduce post-operative GI symptoms or food intolerance compared to LSG alone. Patients with omentopexy did require more anti-nausea medication initially but had no difference in other outcomes. The study concludes that

A 33-year old female presented with abdominal pain, dysuria, and fever. Ultrasound and X-ray revealed a bladder calculus formed around an intrauterine contraceptive device (IUCD) that had migrated from the uterus into the bladder. Such IUCD migration is a rare but known complication, and the device must be removed to prevent further complications like stone formation. Cystoscopy confirmed the presence of the IUCD within the bladder, which was then removed surgically.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
