The Australian Transport Safety Bureau found that the incident was caused by non-conforming oil feed stub pipes within the engine's assembly, which resulted in a fatigue crack and subsequent internal fire. Following the incident, measures were taken by Rolls-Royce and regulatory authorities to ensure the safety of similar engines through identifying and managing defective parts and updates to the engine control software.
