Field Operations personnel are required to submit an Employee Contact Information Form to their supervisor within one week of any change to their address, phone number, education, training, or position. The form collects the employee's name, title, supervisor, department, contact numbers, residence address, emergency contact information, and languages spoken. Once completed, the form is forwarded to the Director's Secretary and Human Resources to update the confidential Employee Emergency Contact List.

I. PURPOSE
To establish a current list of home address and telephone numbers of Field Operations
personnel in order to maintain 24 hour, seven day a week staffing capabilities and the
notification of personnel in case of disasters which would require animal control services.
This also provides personnel a method to update any change in their education, training
or position.
II. POLICY
All Field Operations employees will, within one week of a change of address or
telephone number, supply their immediate supervisor with the completed Employee
Contact Information Form notifying him/her of the change.
NOTE: Message telephone numbers or the address and phone numbers of relatives will not be
accepted as compliance with this policy.

1. Employee Name: Enter the employee’s name Date form completed.
2. Work Title: Enter the employee’s full job classification title (i.e. Animal Control
Officer, Senior Animal Control Officer, Dispatch Services Operator).
3. Supervisor: Enter the employee’s supervisor’s first initial and last name.
4. Section/Department: Enter Field Operations for the section and OC Animal Care
for the department.
5. Office Number: Enter the employee’s contact phone number at the office. Staff
assigned to field duties will enter the Dispatch line. Staff assigned to
administrative duties will provide their direct number.
6. Residence Address: Enter the employee’s address including the street address,
city, state and zip code. If the employee’s mailing address is different than where
he/she resides, (e.g., P.O. Box), the employee will note that information in
addition to his/her place of residence.
7. Phone Number: Enter the employee’s home phone number.
8. Emergency Contact Name: Enter the name of a contact person to call in the event
of an emergency.
9. Emergency Contact Phone Number: Enter the phone number of the employee’s
emergency contact person.
10. Bilingual: Enter any languages other than English the employee is fluent in. If
English is the only fluent language, enter N/A.
Once the form is completed and received by the employees’ supervisor, the form is to be
forwarded to the Director’s Secretary and Human Resources for processing. The
information will be added to the Employee Emergency Contact List located in the Z:
drive under Employee Emergency. This list is to be available to Staff Specialist and
above only and is not to be printed and/or distributed to staff.
