In 2010, over 3 million individuals in Illinois were enrolled in Medicaid, accounting for about a quarter of the state's population. While the majority of enrollees had claims that year, a small percentage—around 3%—accounted for half of all Medicaid funds spent, and just 0.14% accounted for one-tenth of funds. The ACA was expected to add over 500,000 more enrollees by 2014, most living below 133% of the federal poverty line, with the state receiving a 90% match from the federal government. Supportive housing interventions were predicted to significantly cut Medicaid costs as enrollment and spending increased.

The Washington Low Income Housing Alliance created a federal advocacy program to expand their policy and advocacy work beyond the state level. They took several steps: finding an underserved niche in federal advocacy; securing funding; hiring staff; engaging board and membership connections; and working to engage their membership and develop relationships with congressional representatives. The Alliance provides information and recognition to elected officials and keeps members informed through educational events and updates. They have continued developing this work through newsletters, events, and improving existing efforts.

In 2010, over 33,000 people living with HIV/AIDS (PLWHA) were diagnosed in Illinois, with approximately 38% enrolled in Medicaid. By 2014, diagnoses increased to at least 40,000 PLWHA, with over two-thirds enrolled in Medicaid. In 2010, 12,718 PLWHA enrolled in Medicaid used $237 million annually, with 1,417 individuals (11%) accounting for half that amount and 86 individuals (0.69%) accounting for one-tenth or $24 million, averaging $290,000 each. Supportive housing interventions were projected to significantly reduce Medicaid costs as enrollment and spending increased by 2014.

To employers the cost of healthcare is now a business issue and this talk is about what one large buyer IBM did to drive transformation via broad coalition with other large employers to form the Patient Centered Medical Home movement and the covenant between buyer and provider away from the garbage we now buy episodic uncoordinated disintegrated care. In the change of convenient conversation we have worked with the Primary care providers to give us coordinated, integrated, accessible and compressive care with a set of principles know as the Patient centered medical home.
A Patient Centered Medical Home (PCMH) happens when primary care healers keeping that core healing relationship with their patients step up to become specialists in Family and Community Medicine. The move is to the discipline of leading a team that delivers population health management, patent centered prevention, care that is coordination, comprehensive accessible 24/7 and integrated across a deliver system. PCMH happens when the specialists in Family and Community Medicine wake up every morning and ask the question how will my team improve the health of my community today?
All over the world three huge factors are in play that is driving the concept of Patient Centered Medical Home. They are:
1) Cost and demography
2) Information technology and data (information that is actionable will equal a demand for accountability by the payer or buyer of the care)
3) Consumer demand to engage healthcare differently (at least as well as they can their bank- on line) have a question about lab results why not e-mail?
But at its core it is a move toward integration of a healing relationship in primary care and population management all at the point of care with the tools to do just that.
Medical homes 2010
This document discusses integrating behavioral health into patient-centered medical homes. A 2017 survey by Health Catalyst shows that despite uncertainty about the future of the Affordable Care Act, 80 percent of healthcare executives have not paused or otherwise changed their population health management strategy. 68 percent said that PHM is “very important” to their healthcare delivery strategy, while fewer than 3 percent said it was not important at all. The results show that executives view the move to value-based care as inevitable, and they view a PHM strategy as an integral part of their future efforts.

Avenues for Homeless Youth
Avenues for Homeless Youth operates four programs in the Twin Cities that provide shelter and transitional housing for over 200 homeless youth per year. The programs include a shelter in North Minneapolis, as well as GLBT, suburban, and Minneapolis host home programs. Host homes provide a safe, stable transitional housing option at 50% lower cost than congregate housing. They aim to build long-term supportive relationships critical for youth success. The host home model places homeless youth with volunteer community members who are trained and supported by program managers.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).
