This document discusses integrating behavioral health into patient-centered medical homes. It notes that the Affordable Care Act established a medical home pilot program that allows states to enroll Medicaid beneficiaries with chronic conditions, including mental illness and substance use disorders, into medical homes beginning in 2011. States that apply and receive approval will receive a 90% federal match for medical home services provided to beneficiaries through the pilot program. The patient-centered medical home model emphasizes care structured around each patient's needs, so behavioral health must be included to fully address a patient's health. Examples of initiatives integrating behavioral health and primary care are provided from several states.
