2. CC:ปวดเข่าซ้าย5 วัน
• 3 wk PTA มีไข้ ปวด บวม แดง ร้อน ที่เข่าข้างซ้าย ไม่ปวดข้ออื่น ปวดตลอด
วัน ปวด PS 7/10
• 1 wk PTA อาการปวดเป็นมากขึ้น เข่าบวมแดงมากขึ้น ยังมีไข้อยู่ เดินไม่ได้
เนื่องจากปวดมาก ปวด PS 10/10 ไป รพช.

3. ที่ รพช.
• Lt knee swelling tender ballotment positive
• Arthrocentesis
• yellow fluid 10 ml
• WBC 89,030 (N 99 L 1)
• RBC 30,000
• Crystal positive
• Start Ceftriaxone 2 g IV OD
• Refer to รพ.มหาราช

4. รพ.มหาราช
• Refer กลับ Dx. CPPD
• ให้ continue Ceftriaxone จนครบ 7 Days

5. ที่รพช.
• Ceftriaxone 2 g IV OD ต่อ รวม 7 วัน
• อาการไม่ดีขึ้น ยังมีไข้
• Culture ขึ้น Enterococcus spp.
• จึง refer มา รพ.มหาราช

• Personal Hx.
• No smoking
• No alcohol drinking
• ปฏิเสธประวัติฉีดยาเข้าเส้น/การใช้สารเสพติด

7. Physical Examination
GA : alert , good conscious
V/S : T 36.7 , BP 172/82 , RR 20 , HR 100 , SpO2 100%
HEENT : not pale conjunctivae , anicteric sclerae
Heart : pulse full regular , normal S1,S2 no murmur
Lung : clear, normal breath sound , equal both lungs
Abdomen : Soft , not tender , no palpable mass

along joint line , limit
ROM due to pain ,
ballotment positive

• ลด bacterial load
• repeated arthrocentesis
• arthroscopic larvage
• arthrotomy
• empirical antibiotic
• Rehabilitation
• Supportive
• Pain control
- Very turbid pus
- Deep joint e.g. hip and shoulder joints
- Patient’s cooperation
- increasing amount of pus from every day
aspiration
- no clinicalresponses after 48 hr.
conservative treatment and /or persistently
high neutrophilcount >25,000 per ml.
- Osteomyelitisis a complication
- Present of loculation

• early degenerative arthritis
