2. Introduction
Cause and effect diagram can be called as Ishikawa or Fish bone diagram
Fish bone diagrams are typically worked left to right, with each large bone
sub branching smaller bone in more detail.
Design of this diagram looks much like a skeleton of fish
This diagram consists Main, primary causes secondary causes and effect
It combines brain storming with mind map pushes to all the possible causes of
problem rather than just one more obvious
It uncovers the bottle neck in process
It’s straight forward and easy to learn visual tool
It promotes system thinking through visual linkages
It prioritize further analysis and corrective actions
Fish bone diagram is used in analyse the phase of “Six Sigma” DMAIC(Define,
Measure, analyse, improve and control) approach problem solving
4. Three root causes
Casualness of students towards the lectures because management and
lecturer do not take any strict action against the late comers
Availability of internet 24 hours in hostels is also a main root cause because
students use internet up to late night and cannot wake up early in the
morning
Heavy rush and low service rate which are bottleneck in mess operations.
5. Recommended solutions to the root causes
Personal problems
Students should practice 5S for the accessories and books
Students should set alarm everyday to wake up early in the morning
Students should do exercise daily so that they feel fresh and wake up early in the morning
Students should take lunch and dinner on respective allotted timings
Facilities
Number of workers in mess should be increased so that service rate can be increased
Quality of the food should be improved otherwise students will order for some other items
which can cause delay for the class as the time allotted for the mess is limited
6. Students should not be allowed to sit in the mess after finishing their lunch and
breakfast
There should be the separate slots of mess timings for the each branch so that
rush will not be there
Currently mess is at another place from the hostel. it should be in the respective
hostels so that it will distribute the crowd in Nitie 3 hostels , which will
eventually reduces time of having breakfast, lunch and dinner
One service operator should be there in college for lift because lift undergone
breakdown frequently
Less number of washroom are there in new hostels. so there should be more
number of the washrooms
Sufficient amount of the hot water should be provided in the morning
7. Management
Management should take strict against the late comers
Management should distribute the circular among the students which
includes strict instructions about the discipline
Biometric attendance should be provided which will avoid the proxy
Management should also give instructions to the professor that they should
not take late comers leniently

How to ask questions by using why why logic to the existing situation
How to analyse the causes and gathering the information from the ground level
