PROGRAM SUMMARY:
In 2006, initiated by the Chinese University of Hong Kong, further supported by MSD, a web-based
JADE Program was developed.The Program uses information technology to create a virtual platform
to facilitate both health care providers and people with diabetes to adhere to evidenced-based
protocol for diabetes management, using a multidisciplinary approach. The JADEinformation can
be stored, managed and analysed for bench-marking, continuous quality improvement, education
and research purposes.
In Hanoi, the Program was carried out in 10 units ofhospitals, clinics with approximately 2300
patients (of the total 30 units with approximately 6800 patients in Vietnam).
KEY RESPONSIBILITIES :
- Conducts Disease Assessment on Diabetes patients at basic.
- Validating accuracy of inputted patient data.
- Reducing and eliminating the defaults of inputted patient data.
- Support on Nurse training to assessment/putting data on portal, patient education on disease.
- Educate Diabetes patients on Disease and treatment.
- Partnering with patient education program managers to ensure that accurate patient data
is submitted and managed.
- Evaluating the data and implementing.
- Develop marketing plans for each year/month.
- Build website content, find and update website content.
- Present products at the workshop.
- Training members of Marketing group about specialized knowledge that related to products.
- Take care of customer directly through Call center for Customer Consultant.
- Write the product PR.
