• Vision: เป็นโรงพยำบำลที่มุ่งคุณภำพสูงสุดในกำรรักษำพยำบำลระดับตติยภูมิ
• IT Environment
– HIS เน้นเรื่องกำรเงิน กำรเบิกจ่ำยค่ำรักษำพยำบำล มีระบบทะเบียนผู้ป่วย (Master
Patient Index), ระบบ Admission-Discharge-Transfer (ADT), ระบบนัด
– แต่ละจุดบริกำรมี computer terminal เพื่อสนับสนุน basic workflow เพื่อกำร
บริกำรผู้ป่วย
• People
– บุคลำกรมีอำยุเฉลี่ย 45 ปี (range 25-59), แผนก IT มีลูกจ้ำงชั่วครำว 5 คน
– แพทย์ใส่ใจเรื่องผู้ป่วย รวมตัวกันเหนียวแน่น มีแพทย์ที่เป็นแกนนำ 4-5 คน
– ควำมสัมพันธ์ระหว่ำงแพทย์กับวิชำชีพอื่นเป็นไปด้วยดี แต่มักวิพำกษ์วิจำรณ์ผู้บริหำรว่ำ
ไม่ให้ควำมสำคัญกับแพทย์และกำรบริกำร ไม่เคยลงมำดูกำรทำงำน

• โรงพยำบำลเอกชน 200 เตียง ในภำคใต้ จังหวัดที่เป็นแหล่งท่องเที่ยวสำคัญ เป็น
ส่วนหนึ่งของเครือ รพ. ขนำดใหญ่ ฐำนะ รพ. ดี มี รพ.เอกชนในพื้นที่ ~ 2-3 แห่ง
• Vision: เป็นโรงพยำบำล High Tech High Touch ชั้นนำของประเทศ
• IT Environment
– HIS มีระบบทะเบียนผู้ป่วย (Master Patient Index), ระบบ Admission-Discharge-
Transfer (ADT), Electronic Health Records (EHRs), Computerized Physician Order
Entry (CPOE) แต่ยังมีระบบ Clinical Decision Support System (CDS) จำกัด ระบบ HIS
ช่วยลด waiting time และเพิ่มประสิทธิภำพกำรบริกำรอย่ำงเต็มที่
– มีระบบลูกค้ำสัมพันธ์ (Customer Relationship Management - CRM)
ที่ effective มำก โดยเป็น standard out-of-the-box implementation เป็นส่วนใหญ่
• People
– บุคลำกรมีอำยุเฉลี่ย 37 ปี (range 20-57) แผนก IT เข้มแข็ง
– แพทย์ไม่ค่อยมี interaction กับบุคลำกรอื่น, รำยได้เป็นแรงดึงดูดหลัก
– ผู้บริหำรได้รับกำรยอมรับจำกบุคลำกรทุกวิชำชีพว่ำมีวิสัยทัศน์และบริหำรงำนได้ดี

• โรงพยำบำลมหำวิทยำลัย 900 เตียง ใน กทม. สังกัดมหำวิทยำลัยของรัฐ มี รพ. ในบริเวณใกล้เคียง > 8
รพ. งบประมำณค่อนข้ำงมำก มีพันธกิจหลำยด้ำนที่สำคัญพอๆ กัน
• Vision: เป็นโรงพยำบำลชั้นนำของภูมิภำคเอเชียที่มีควำมเป็นเลิศในด้ำนบริกำร กำรศึกษำ และวิจัย
• IT Environment
– เป็น รพ.แรกๆ ที่มี HIS ซึ่งพัฒนำเอง และต่อยอดจำกระบบ Master Patient Index, ระบบ ADT
ไปสู่ CPOE ในผู้ป่วยใน (แต่ยังขำด CPOE ผู้ป่วยนอก ระบบ EHRs และระบบ CDS ที่เข้มแข็ง ระบบ
HIS เข้ำกับ workflow ของ รพ. เป็นอย่ำงดี
– ปัจจุบัน ระบบ HIS ยังใช้เทคโนโลยี Visual FoxPro เหมือนกับช่วงที่พัฒนำใหม่ๆ (20 ปีก่อน) เป็น
หลัก มีปัญหำเสถียรภำพของระบบบ้ำง มีกำรนำเทคโนโลยีใหม่ๆ มำใช้อย่ำงช้ำๆ
– มีระบบ ERP ที่ cutting edge และได้ customize เพื่อให้เข้ำกับองค์กร ซึ่งมี requirement
dynamics สูง integrate กับ HIS ได้ดี และมี business intelligence ที่เป็นประโยชน์กับผู้บริหำร
เป็นอย่ำงมำก และมีทีมที่มีควำมคล่องตัวสูงในกำรปรับแก้ระบบ
• People
– บุคลำกรมีอำยุเฉลี่ย 40 ปี (range 20-65), แผนก IT มีทั้งบุคลำกรใหม่และที่เคยพัฒนำระบบ
HIS ตั้งแต่แรกเริ่ม
– แพทย์มีควำมเป็นตัวของตัวเองสูง, มักทำงำนเอกชนด้วย, มี turn-over rate สูง
– พยำบำลและวิชำชีพอื่นมักมองว่ำแพทย์คืออภิสิทธิ์ชน และมีเรื่องถกเถียงกันบ่อยๆ
