Countermeasures at each level of WHY Process – Any material specification change to be approved by user dept. Refurbishment – replace gasket Asset care – check for leaks, flange tightened or not Stain – Cleaning standard for the shop-floor What’s the benefit of doing 5-Whys? – this same root cause, could have many other effects… Ask “what other bad things could have happened due to root cause” – buy fire extinguisher, safety belts, PRVs meeting IS specs… things could go wrong… 5-Why helps systemic level improvements that are more lasting and more pervasive If I clean spill, the benefits will remain for 5 – 10 min If I tighten flange, the benefit will remain for 2 hours If I replace gasket, the benefit will remain for 1 week If I change back to haathi-chaap, all gasket related problems will be solved If I attack the root cause all purchase-related problems can be solved HOW IS 5-WHY DONE? 1. At the Gemba – see at the workplace 2. How many why’s? – till it is in our control
Emphasize phenomenon – take an example which has happened in the past 2-3 days If the problem persists after taking counter-measures, would be because of a different reason Difference between fishbone and 5 WHY (if many causes are being put up) Focus on the process with seriousness so that the whys are discussed objectively and fingers are not pointed Involvement of people from maintenance and operations while doing 5 WHY is to be stressed The effectiveness of countermeasures is to be stressed
