PLANNING
1. Assemble equipmentandsupplies:

Vial or ampule of the correct medication
Sterile 1-mL syringe calibrated into hundredths of a
milliliter (i.e., tuberculin syringe) and a #25- to #27-
gauge safety needle that is 1/4 to 5/8 inch long
Alcohol swabs
2x2 sterile gauze square (optional)
Clean gloves (according to agency protocol)
Bandage (optional)
Epinephrine on hand in case of allergic anaphylactic
reaction
IMPLEMENTATION
1. 1. Checkthe Chart / Record
• Checkthe label onthe medicationcarefullyagainstthe Chart
to make sure that the correct medicationisbeingprepared.
• Followthe three checksforadministeringthe medication
and dose.Readthe label onthe medication(1) whenitis
takenfromthe medicationcart,(2) before withdrawingthe
medication,and(3) afterwithdrawingthe medication.
• Confirmthatthe dose iscorrect.

2. Organize the equipment
3. Performhand hygiene andobserve other appropriate
infectioncontrol procedures(e.g.,clean gloves)
4. Prepare the medicationfrom the ampule or vial for drug
withdrawal.
5. Prepare the client
• Priorto performingthe procedure,introduce self andverify
the client’sidentityusingagencyprotocol
6. Provide for clientprivacy.
7. Selectand clean the site.
• Selectasite (e.g.,the forearmabouta hand’swidthabove
the wristand three or fourfingerwidthsbelow the antecubital
space).
• Avoidusingsitesthatare tender,inflamed,orswollenand
those that have lesions.
• Applyglovesasindicatedbyagencypolicy.
• Cleanse the skinatthe site usinga firmcircularmotion
startingat the centerandwideningthe circle outward.Allow
the area to dry thoroughly.
8. Prepare the syringe for the injection.
• Remove the needle capwhile waitingforthe antisepticto
dry.
• Expel anyairbubblesfromthe syringe.Small bubblesthat
adhere tothe plungerare of no consequence.
• Grasp the syringe inyourdominanthand,close tothe hub,
holdingitbetweenthumbandforefinger.Holdthe needle
almostparallel tothe skinsurface,withthe bevel of the
needle up.
9. Injectthe fluid
• Withthe nondominanthand,pull the skinatthe site until it
istaut. For example,if usingthe ventral forearm, graspthe
client’sdorsal forearmandgentlypull ittotightenthe ventral
skin.
Insertthe tipof the needle farenoughtoplace the bevel
throughthe epidermisintothe dermis.The outlineof the
bevel shouldbe visible underthe skinsurface.
• Stabilize the syringe andneedle.Injectthe medication
carefullyandslowlysothatitproducesa small wheal onthe
skin.
• Withdrawthe needle quicklyatthe same angle atwhichit
was inserted.Activate the needlesafetydevice.Applya
bandage if indicated.
• Do notmassage the area.
• Dispose of the syringe andneedleintothe sharpscontainer.
