Uses of Tiova Rotacaps
Tiova Rotacaps (Generic Tiotropium Bromide Inhalation Powder) are
used for the long-term, once-daily, maintenance treatment of
bronchospasm associated with chronic obstructive pulmonary disease
(COPD), including chronic bronchitis and emphysema. COPD is a lung
disease that also includes chronic bronchitis (swelling of the tubes leading
to the lungs) and emphysema (damage to the air sacs in the lungs). It is
also used as maintenance treatment for asthma in adults and children 6
years of age and older.
Tiotropium belongs to the family of medicines known as bronchodilators.
Bronchodilators are breathed in through the mouth to help open up the
bronchial tubes (air passages) in the lungs. It is taken by inhalation (an
inhaler) and will increase the flow of air to the lungs.

The recommended dosage of Tiova Rotacaps is one rotacap inhaled once
daily, using the Cipla Rotahaler/ Revolizer.
No dosage adjustment is required for geriatric, hepatically-impaired, or
renally-impaired patients. However, patients with moderate to severe
renal impairment, who are being treated with Tiova Rotacaps, should be
monitored closely.
Tiova Rotacaps are for inhalation only and must not be swallowed.

Warnings
Tiova Rotacaps (Tiotropium Bromide Inhalation Powder) should not be
used as a rescue medication.
Discontinue Tiova Rotacaps at once and consider alternatives in case of
immediate hypersensitivity reactions, including angioedema, occur. Use
with caution in patients with severe hypersensitivity to milk proteins.
Stop using Tiova Rotacaps and consider other treatments if paradoxical
bronchospasm occurs.
Worsening of narrow-angle glaucoma may occur. Use with caution in
patients with narrow-angle glaucoma and instruct patients to consult a
doctor immediately if this occurs.
Worsening of urinary retention may occur. Use with caution in patients
with prostatic hyperplasia or bladder-neck obstruction and instruct
patients to consult a physician immediately in case this occurs. year.

Warnings
Inhaled medicines such as Tiotropium Bromide Inhalation powder capsules
(Tiova Rotacaps 18 microgram) may cause tightness of the chest, coughing,
wheezing or breathlessness immediately after inhalation. If this occurs,
patients should be advised to consult their doctor immediately.
As a predominantly renally excreted medicine, patients with moderate to severe
renal impairment (creatinine clearance of <60 mL/min) treated with Tiova
Rotacaps must be monitored closely for anticholinergic side effects.
The occurrence of dizziness, blurred vision, or headache may influence the ability
to drive and use machinery.
Patients must inform their doctor in case they have suffered from a myocardial
infarction during the last 6 months or from any unstable or life threatening
irregular heart beat or severe heart failure within the past year.

Not known (frequency cannot be estimated from the available data): Depletion of body
water (dehydration), Tooth decay (dental caries), Severe allergic reaction (anaphylactic

Overdose
High doses of Tiova Rotacaps (Tiotropium Bromide Inhalation Powder)
could lead to anticholinergic signs and symptoms. However, there were
no systemic anticholinergic adverse effects following a single inhaled dose
of up to 282 mcg Tiotropium in 6 healthy volunteers.
In a study of 12 healthy volunteers, bilateral conjunctivitis and dry mouth
were seen following repeated once-daily inhalation of 141 mcg of
Tiotropium.
Acute intoxication by inadvertent oral ingestion of Tiova Rotacaps is
unlikely since it is not well-absorbed systemically.

Overdose
A case of overdose has been reported from post-marketing experience.
A female patient was reported to have inhaled 30 capsules over a 2.5
day period, and developed altered mental status, tremors, abdominal
pain, and severe constipation.

Inhalation Powder) was discontinued, and the constipation was treated
with an enema. The patient recovered and was discharged on the same
day.

is classified by the US FDA as Pregnancy Category C.

effects with respect to reproductive toxicity at clinically
