2. 6.2.11 The Provincial Director assigns the TESDA Representative
(TESDA-OP-CO-05-F29) among the TESDA employees or
TESDA-authorized individuals. The TESDA Representative
oversees the conduct of assessment and monitors whether the
assessment process is in accordance with the requirements as
identified in the Competency Assessment Tools.
6.2.14. The TESDA Representative receives the Assessment Package from
the PO CAC Focal person.
6.3.2. The TESDA Representative should be at the Assessment Center/Venue
at least thirty (30) minutes ahead of schedule.

3. 6.3.3. The TESDA Representative hands-over the Assessment Package to the
Competency Assessor:
a. Specific Instructions to the Candidates corresponding to the
number of candidates
b. Competency Assessment Results Summary (CARS)
corresponding to the number of candidates.
c. Rating Sheets corresponding to the number of candidates; and
d. One (1) copy of Assessor’s Guide
6.3.5. The TESDA Representative checks the Attendance Sheet vis-à-vis
the actual number of candidates.
6.3.6. The TESDA Representative and the Competency Assessor wear
official ID throughout the conduct of assessment.

5. Step 1 – Establishing the Assessment Context & Purpose of
Assessment
The Competency Assessor:
6.3.7.1 Reviews the Qualification or units of competency
assessment to be assessed.
6.3.7.2 Checks the environment where the assessment will
take place complies with the requirements and purpose
of assessment

8. Step 3 – Preparing the Candidate
The Competency Assessor:
6.3.7.4 Checks attendance and verifies identity of candidates
vis-à-vis the Admission Slips and requests candidates
to sign on the Attendance Sheet (TESDA-OP-CO-05-
F31).
If the candidate is absent on the date of assess-
ment, it shall be so indicated in the Attendance Sheet;
he/she shall forfeit the assessment fee and his/her
name shall not be reflected in the RWAC.

15. 6.3.7.18 The Competency Assessor accomplishes the CARS
(TESDA-OP-QSO-02-F09)
a. Recommends issuance of NC if candidate is found

18. Post Assessment
6.3.8 The TESDA Representative requests at least two (2)
Candidates and the AC Manager to accomplish the
Competency Assessor’s Performance Evaluation
Instrument (PEI) (TESDA-OP-CO-04-F37) at the end
of each assessment.
6.3.9 The TESDA Representative provides a copy of the
accomplished PEI (TESDA-OP-CO-04-F37) to the
Accredited Competency Assessor.

19. Post Assessment
6.3.10 The TESDA Representative prepares Report on the
Assessment Proceedings (TESDA-SOP-CO-05-F34)
at the end of each assessment.

20. 6.4 Review and issuance of assessment results
6.4.1. Upon completion of the assessment process, the
Competency Assessor returns to AC Manager the
following assessment-related documents:
a. Accomplished Application Forms with picture
b. Accomplished Self-Assessment Guide
c. Accomplished Attendance Sheet
e. Accomplished CARS
6.4.2. The AC Manager reviews the assessment related
documents.

6.5.1 The Competency Assessor returns to the TESDA
Representative the following:
a. Assessor’s Guide
b. Specific Instructions to the Candidate
6.5.2. The TESDA Representative checks consistency of
entries in the result column in the attendance sheet
(TESDA-OP-CO-05-F31) against the Rating Sheets
and CARS, after which he/she signs the Attendance
Sheet.

23. 6.5 Return of assessment documents and submission
of reports
6.5.3 The AC Processing Officer prints and signs three (3)
copies of RWAC (TESDA-OP-CO-08-F33) generated
in the T2MIS.
6.5.4 The Competency Assessor and AC Manager check
the consistency of entries in the assessment result
column in the RWAC against the Rating Sheet and
CARS, after which they sign the RWAC.
