What is Life Links?
• Planned preventative outreach service
• Targets people and their carers who may be on the brink of physical
dependence or social isolation
• Specific focus on those currently unknown to health and care services
• Provides self-help advice and referrals to local non-statutory services
• Promotes independence and good health and wellbeing
• Purpose is to improve health and wellbeing and prevent unnecessary access
to health and care services
• Evaluation tool will measure against specific, desired outcomes

Service levels
Level 1 – Primary prevention/promoting wellbeing
• This is aimed at people who have little or no social care needs or symptoms
of illness, and their carers/family
• The focus is on maintaining independence and good health and promoting
wellbeing
• Interventions include combating ageism, providing universal access to good
quality information and advice, creating safer neighbourhoods, promoting
healthy and active lifestyles, and signposting to low level practical support
such as equipment and assistive technologies

Service levels
Level 2 – Secondary prevention/early intervention
• This is aimed at people at risk and their carers and family to halt or slow
down any deterioration and actively seek to improve their situation.
• The focus is on interventions including seeking to reach individuals at risk of
specific health conditions or events (such as strokes or falls) or those who
have existing low level social care needs by providing advice at key places
where they might self-identify e.g. GP surgeries
• Provide the minimum amount of input required to meet outcomes specified
in order to
– Minimise dependency and maximise self-reliance and independence
– Make the best use of available resources to meet community needs
– Minimise the cost to the individual where appropriate.

Evaluation
• Purpose is to improve people’s wellbeing and prevent or delay individuals’
need to access health and care services
• We are developing an evaluation tool in line with a research company
commissioned by Cheshire East Council
• We will measure how our interventions have improved wellbeing through
approved performance indicators
• Example measures:
– Increase in sense of personal wellbeing
– Increase in use of self-help tools
– Increase in use of technology
– Increase in engagement with activities in community
