Why a review of end of life care?
ā€¢ c500,000 people die in England and Wales every year
ā€¢ UK ranked global #1 in The Quality of Death Index*
ā€¢ But quality of care is variable:
ā€¢ 90% of hospices are rated as good or outstanding
ā€¢ 42% of end of life care services in hospitals are
rated inadequate or require improvement
ā€¢ More Care, Less Pathway (review of the Liverpool Care
Pathway) recommended CQC undertake a review to look
at how dying people are treated across various settings.

Why did we focus on inequalities?
ā€¢ Most people die of conditions
other than cancer
ā€¢ Two thirds of people are aged
over 75 when they die
ā€¢ The 2008 End of Life Care
Strategy* outlined a vision of
high quality care for allā€¦
ā€¢ But inequality in end of life care
persists, and many do not
experience the good quality,
personalised care they have a
right to expect

What did we want to find out?

experiencing good quality, joined up care at
the end of life?
ā€¢ What good practice can others learn from local
level?
ā€¢ What action can national and local
stakeholders take in the next five years to
address inequalities?
On whom did we focus?
ā€¢ People with conditions other than cancer
ā€¢ Older people
ā€¢ People with dementia
ā€¢ People from Black, Asian and minority ethnic groups
ā€¢ People who are lesbian, gay, bisexual or transgender
ā€¢ People with a learning disability
ā€¢ People with a mental health condition
ā€¢ People who are in secure and detained settings
ā€¢ Gypsies and Travellers
ā€¢ People who are homeless

How did we carry out the review?
ā€¢ Evidence review - national data on quality of care and a
literature review
ā€¢ National Council for Palliative Care, Race Equality
Foundation and National Voices ā€“ to talk to people
about their experiences
ā€¢ Online survey of people with EOLC experience
ā€¢ Talked to commissioners, health and care staff and
people with experience, and reviewed peopleā€™s case
notes. We looked at:
ā€¢ Identification and communication
ā€¢ Coordination of and access to care
ā€¢ Care in the last days and hours of life

What did we find about peopleā€™s
experience?
ā€¢ Difficulty identifying the last 12
months of life (esp. for those
with conditions other than
cancer)
ā€¢ Conversations about EOLC not
always early enough
ā€¢ Communication hard for
everyone, including health and
care staff
ā€¢ Lack of coordination of care

What did we find about peopleā€™s
experience?
ā€¢ The needs of some groups are particularly overlooked,
including:
ā€¢ people with a mental health condition
ā€¢ people with a learning disability
ā€¢ people who are homeless
ā€¢ Gypsies and Travellers
ā€¢ People from these groups are often excluded from wider
health services

What did we find about local health
and care systems?
ā€¢ Commissioners and providers donā€™t always understand
EOLC needs of people from different groups
ā€¢ Equality-led approach is needed to meet peopleā€™s
individual needs
ā€¢ Electronic Palliative Care Coordination Systems are
not yet effective locally, but care coordinators support
improved coordination
ā€¢ Some hospices engaging local communities,
delivering care based on individual need, and
supporting other local health and care services to
do the same

recording their wishes and treatment preferences

  • 2. About CQC: our purpose The Care Quality Commission is the independent regulator of health and adult social care in England. We make sure health and social care services provide people with safe, effective, compassionate, high-quality care and we encourage care services to improve. 2

  • 4. 4 Why a review of end of life care? ā€¢ c500,000 people die in England and Wales every year ā€¢ UK ranked global #1 in The Quality of Death Index* ā€¢ But quality of care is variable: ā€¢ 90% of hospices are rated as good or outstanding ā€¢ 42% of end of life care services in hospitals are rated inadequate or require improvement ā€¢ More Care, Less Pathway (review of the Liverpool Care Pathway) recommended CQC undertake a review to look at how dying people are treated across various settings. *The Economist Intelligence Unit, 2015.

Why did we focus on inequalities? ā€¢ Most people die of conditions other than cancer ā€¢ Two thirds of people are aged over 75 when they die ā€¢ The 2008 End of Life Care Strategy* outlined a vision of high quality care for allā€¦ ā€¢ But inequality in end of life care persists, and many do not experience the good quality, personalised care they have a right to expect 5 ā€œā€¦irrespective of age, gender, ethnicity, religious belief, disability, sexual orientation, diagnosis or socioeconomic deprivation.ā€ *Department of Health, 2008.

What did we want to find out? 6 ā€¢ What factors prevent people from experiencing good quality, joined up care at the end of life? ā€¢ What good practice can others learn from local level? ā€¢ What action can national and local stakeholders take in the next five years to address inequalities?

On whom did we focus? ā€¢ People with conditions other than cancer ā€¢ Older people ā€¢ People with dementia ā€¢ People from Black, Asian and minority ethnic groups ā€¢ People who are lesbian, gay, bisexual or transgender ā€¢ People with a learning disability ā€¢ People with a mental health condition ā€¢ People who are in secure and detained settings ā€¢ Gypsies and Travellers ā€¢ People who are homeless 7

How did we carry out the review? ā€¢ Evidence review - national data on quality of care and a literature review ā€¢ National Council for Palliative Care, Race Equality Foundation and National Voices ā€“ to talk to people about their experiences ā€¢ Online survey of people with EOLC experience ā€¢ Talked to commissioners, health and care staff and people with experience, and reviewed peopleā€™s case notes. We looked at: ā€¢ Identification and communication ā€¢ Coordination of and access to care ā€¢ Care in the last days and hours of life 8

What did we find about peopleā€™s experience? ā€¢ Difficulty identifying the last 12 months of life (esp. for those with conditions other than cancer) ā€¢ Conversations about EOLC not always early enough ā€¢ Communication hard for everyone, including health and care staff ā€¢ Lack of coordination of care 9 ā€œIt is hard to have the conversations we all need to have with family, friends, colleagues and in the media, but this has to happen...ā€ ā€¢ Lack of understanding of peopleā€™s individual needs ā€¢ Good communication, making choices and planning are important for getting the right care

What did we find about peopleā€™s experience? ā€¢ The needs of some groups are particularly overlooked, including: ā€¢ people with a mental health condition ā€¢ people with a learning disability ā€¢ people who are homeless ā€¢ Gypsies and Travellers ā€¢ People from these groups are often excluded from wider health services 10 ā€œWe couldnā€™t bring him back, we couldnā€™t do it how we wanted to do it and they didnā€™t understand, they didnā€™t think it was a big thing.ā€

What did we find about local health and care systems? ā€¢ Quality of end of life care is variable ā€“ hospices have highest ratings overall ā€¢ Examples of outstanding, responsive care in all settings ā€“ esp. those that supported improvement in personalised EOLC ā€¢ Good practice associated with a collaborative leadership approach, with a shared focus on addressing inequality and meeting peopleā€™s individual needs 11
