2.  Sub center action plan is a very important tool of
work at sub center. It provides information about
activities planned for each village under sub
health center and estimation of all services
provided by the sub center in a particular period.

3.  Sub center action plan is defined as a written
document which contains information about the
activities to be performed in a specified time
frame indicating resources, time table and venue
for each action to be performed.

4.  preparation of map- First step is to prepare a map
of the area covered by the sub center. It should
include all villages, approach roads, important
places such as schools, community centers, govt.
offices and places of worships.

5.  Survey of the area- carry out a detailed survey of
each village in your area to collect relevant
information.
 Conducting meetings- conduct meetings ith health
team regarding action plan and include suggestions
of team.

6.  Identify high risk groups- Identification of high risk
group ill help in setting priorities of the services.
 Setting priority- as per need of the community you
should set priority of the services in each village.
