Accidents are rarely caused by a dramatic failure
of equipment and are mainly caused due to
human failure. Even an apparent failure of
equipment may be due to common factor in
failing to recognize signs of impending trouble.

the worker.

A condition in the work place that is likely to
cause property damage or injury.
